摘 要:目的:对比新研制的钛基钽涂层人工种植体植入犬下颌骨和股骨不同部位的植体骨结合能力差异。方法:选5只成年健康比格犬,拔除双侧下颌第一磨牙,愈合3个月建立缺牙模型。按临床种植体植入手术操作步骤,分别于下颌骨和股骨中上段植入钛钽种植体,每只犬双侧颌骨及股骨分别植入各2颗。术后3个月取材,行X线片、Micro-CT扫描及骨密度(BMD)分析、带植体硬组织磨片组织学及Goldner三色法染色检查,Image J图像分析新骨形成面积。结果:钛钽人工种植体植入犬体内3个月,X线示两组植体均与骨结合紧密,植体骨界面无低密度阴影。Micro-CT扫描显示:颌骨组种植体与骨结合紧密,界面无低密度透射影,局部微结构、密度变化无显著差异;股骨组种植体-骨界面近髓腔段呈低密度线状透射影,Micro-CT骨形态计量分析结果显示:颌骨组BMD为1169±86.39mg/cc,股骨组BMD,754.2±129.4mg/cc,颌骨种植体周围BMD显著高于股骨种植体周围BMD(P<0.05);亚甲蓝-酸性品红染色显示:颌骨组种植体周有红色带状深染新骨形成,植体侧呈指状突入黑色植体螺纹之间,颌骨侧呈波浪与蓝染的颌骨组织移行,植体骨界面密合无间隙。股骨组也有红色深染新骨形成,但新骨带较颌骨组较窄,且植体-骨界面部分不密合,见条状纤维结缔组织样结构。ImageJ图像分析显示:颌骨组新骨形成面积294.4±10.18μm^2,股骨组新骨形成面积108.4±3.208μm^2,新骨形成面积颌骨组显著多于股骨组(P<0.05)。结论:钛基钽涂层人工种植体植入下颌骨较植入股骨更能促进种植体骨结合,本研究为种植体骨结合动物实验模型选择提供了依据。Objective: To explore the difference in osseointegration of the newly developed tantalum coating titanium implant in the mandibular and femur. Method: 5 Beagle dogs with 1-2 years old were selected and the teeth-missing animal model were established by removal of the first molar of bilateral mandibular and experienced a healing period of 3-month. Then, the newly developed tantalum coating titanium implant (Ta/Ti implants) were placed in the mandibular and femurs of the dogs with 2 implants in each lateral respectively according to the routine operative procedure. 3-month post-implanted, the mandibular and the femurs contained Ta/Ti implants were performed detections including X-ray, Micro-CT scan, histo-logical analysis through both methyleneblue-Fuchsine Acid staining and Goldner's trichrome staining. Moreover, Image J software was employed to analyze the area of new-forming bone. Results: 3-month post-implantation, X-rays of the speci-mens showed that no difference was found in the interface between Ta/Ti implant and the mandibular or Ta/Ti implant and the femurs. Furthermore, analysis of Micro-CT in the mandibular group displayed a complete integration in the interface be-tween Ta/Ti implant and bone, while a low density visible gap was found in the femoral group. Bone mineral density (BMD) in the mandibular group was 1169± 86.39mg/cc, which is significantly higher that of in the femurs, 754.2± 129.4mg/cc (P〈0.05). Histological observation revealed that the new bone around the implants grew on the surface of Ta/Ti implant in the mandibu-lar group with red zonal distribution around the implant side and connected with the blue staining mature bone. This red staining bone looked like wave on the surface of bone and intruded finger-like into the screw of the implant. The osse-inte- gration was almost complete and no obvious interval was seen.
