术前血小板分布宽度在子宫内膜癌诊断预测中的价值

摘　　要：目的探讨术前血小板分布宽度(PDW)在子宫内膜癌筛查及诊断预测中的价值。方法纳入2012-2016年中山大学附属第三医院收治的135例子宫内膜癌患者和135例子宫内膜息肉患者,收集其临床资料并行回顾性分析。结果子宫内膜癌患者术前PDW中位值低于子宫内膜息肉患者,差异有统计学意义(P<0.001)。随着FIGO分期增加,术前PDW中位值呈现逐渐下降趋势(P<0.05)。ROC曲线分析示,灵敏度75.9%,特异度60.9%,AUC 0.673(95%CI 0.541~0.806)(P<0.05)。有淋巴结转移、深肌层浸润、淋巴脉管间隙受累、宫颈浸润、附件转移患者术前PDW中位值低于无淋巴结转移、深肌层浸润、淋巴脉管间隙受累、宫颈浸润、附件转移患者,差异有统计学意义(P<0.05)。结论术前PDW数值有助于预测子宫内膜癌高危因素和肿瘤进展程度。Objective To evaluate the value of preoperative Platelet Distribution Width in the diagnosis of Endometrial Cancer. Methods This retrospective study included 135 endometrial cancer patients and 135 endo- metrial polyp patients in the Third Affiliated Hospital of Sun Yat-sen University from 2012 to 2016. The clinical data were retrospectively reviewed and analyzed. Results The median preoperative PDW of endometrial cancer patients was lower than that of endometrial polyp patients （P 〈 0.001 ）. Reduced preoperative PDW was correlated with higher FIGO stage （P 〈 0.05）. ROC curve revealed the sensitivity was 75.9%, the specificity was 60.9%, and AUC was 0.673 （95% CI 0.541 - 0.806） （P 〈 0.05）. Reduced preoperative PDW was correlated with lymph node metastasis, deep muscle infiltration, lymph vascular space involvement, cervical involvement and adnexal involvement （P 〈 0.05 ）. Conclusions Preoperative PDW is a favorable predictor of stage and risk factors of endometrial cancer.
