摘　　要：目的探讨无先兆型偏头痛患者基于前脑岛的功能连接改变及临床意义。方法选取无先兆型偏头痛患者20例及健康对照者20人,以双侧前脑岛为种子点,分析两组间功能连接的差异,对异常连接与患者临床症状的相关性进行分析。结果与健康对照组比较,无先兆型偏头痛组前脑岛与右侧中央后回,双侧中扣带回,右侧额中回,双侧缘上回及右侧颞中回间的功能连接减弱;无功能连接增强的脑区。相关性分析结果显示,这些异常连接与临床症状无关。结论无先兆型偏头痛患者与前脑岛功能连接改变的脑区主要位于痛觉辨别处理区、情感情绪处理区及认知加工区,为研究偏头痛的病理生理机制提供了新的视角。Objective To investigate the change and its clinical significance of functional connectivity （FC） of anterior insular in migraineurs without aura. Methods Twenty migraineurs patients without aura and 20 healthy controls were explored in our study. The differences of resting-state FC based on the bilateral anterior insulars between two groups were analyzed, and the correlations between abnormal FCs and clinical symptoms were calculated. Results Compared with healthy control group, migraineurs without aura group exhibited decreased FCs between anterior insular and right posterior centeral gyms, bilateral middle cingulate gyri, right middle frontal gyrus, bilateral supramarginal gyri and right middle temporal gyrus. No increased FC was found. Correlation analysis showed that these abnormal connections were not associated with clinical symptoms. Conclusions Brain regions showing altered FCs with bilateral anterior insulars in migraineurs are those involved in sensory-discriminative processing of pain, affective-emotional processing, and cognitive processing. And these abnormal FCs may provide new insight into the investigation of pathophysiology for migraine.
