摘  要:目的探讨甲状腺转录因子-1(TTF-1)、表皮生长因子受体(EGFR)、肿瘤浸润性树突状细胞(TIDC)在肺腺癌中的表达情况及其临床意义。方法回顾性收集肺腺癌患者100例,比较肿瘤组织和癌旁组织中EGFR、TTF-1、TIDC表达水平,同时分析肿瘤组织中EGFR、TTF-1、TIDC与肺腺癌患者肿瘤分化程度和TNM分期的关系。结果肿瘤组织中TIDC数密度、MHC-Ⅱ阳性DC、CD54阳性DC均明显低于癌旁组织,TTF-1阳性率、EGFR突变率均明显高于癌旁组织,差异均有统计学意义(P﹤0.01);肿瘤细胞为中低分化及TNM分期为Ⅲ~Ⅳ期患者TIDC数密度、MHC-Ⅱ阳性DC、CD54阳性DC均低于高分化患者及Ⅰ~Ⅱ期患者,TTF-1阳性率、EGFR突变率均高于高分化患者及Ⅰ~Ⅱ期患者,差异均有统计学意义(P﹤0.05)。结论 TIDC在肿瘤组织中表达水平较低,且多为不成熟的调节性DC细胞,TTF-1在肺腺癌中表达水平较高,EGFR突变率增高。EGFR、TTF-1、TIDC均与临床分期和肿瘤细胞分化程度有关。Objective To investigate the expression and clinical significance of thyroid transcription factor-1(TTF-1),epidermal growth factor receptor(EGFR), and tumor infiltrating dendritic cell(TIDC) in lung adenocarcinoma. Method100 cases of lung cancer patients were included in the analysis, of which the expression of EGFR, TTF-1, and IDC in the tumor tissues and adjacent normal tissues were analyzed retrospectively. The relationship between EGFR, TTF-1, TIDC in tumor tissues and differentiation, as well as TNM staging in lung adenocarcinoma was analyzed. Result The TIDC number density and MHC-II positive DC, and CD54 positive DC in tumor tissues were significantly lower than that in adjacent normal tissues, while the positive rate of TTF-1 and mutation rate of EGFR were significantly higher than that in adjacent tissues, the difference was statistically significant(P〈0.01); the TIDC number density and MHC-II positive DC as well as CD54 positive DC in patients with poor and moderate differentiation and TNM stage III-IV were lower compared with those with high differentiation and stage I-II disease, but the positive rate of TTF-1 and mutation rate of EGFR were higher, with statistically significant difference observed(P〈0.05). Conclusion TIDC are with low expression in tumor tissues, mainly are immature regulatory DC cells, while TTF-1 is highly expressed in lung adenocarcinoma, EGFR mutation rate is increased. Overall, EGFR, TTF-1 and TIDC are correlated with the clinical stage and the differentiation of tumor cells.
