熄风通络止痛汤治疗三叉神经痛的疗效及对患者体内P物质和β-内啡肽水平的影响

摘　　要：目的:观察中药汤剂治疗三叉神经痛的效果研究以及对患者体内P物质和β-内啡肽水平的影响。方法:选择2015年1月至2016年12月被我院诊断为原发性三叉神经痛的患者180例,根据excel表格随机数字表法随机分为两组:医药组90例口服卡马西平片治疗,中药组90例采用熄风通络止痛汤治疗,7天为1个疗程,连续治疗4个疗程。比较治疗前后两组患者血浆中P物质和β-内啡肽水平;比较治疗前后两组患者的VAS评分和PSQI评分以及治疗疗效。结果:研究结果显示,中药组的总有效率为94.44%,西药组的总有效率为82.22%,两组比较差异有统计学意义(P<0.05);治疗后两组的P物质水平较治疗前都有所下降,中药组下降幅度更大(P<0.05),治疗后两组的β-内啡肽水平较治疗前均有所提升,中药组的提升幅度更大(P<0.05);两组治疗后VAS评分和OS评分较治疗前均有所降低,治疗后中药组的VAS评分低于西药组(P<0.05)。结论:中药汤剂治疗原发性三叉神经痛可以降低疼痛介质的释放,缓解疼痛,改善睡眠,提高临床治疗效果,值得临床推广应用。Objective： To evaluate the effect of Chinese herb medicine decoction for treating trigeminal neuralgia and its effects on substance P and beta endorphin levels in patients. Methods： 180 cases of patients from January 2015 to December2016 in our hospital were diagnosed as primary trigeminal neuralgia,according to the excel table were randomly divided into two groups： group of oral medicine treatment of 90 cases with Carbamazepine Tablets,the Chinese medicine group of 90 cases with Xifeng Tongluo Decoction in the treatment of pain,7 days for 1 course of treatment,continuous treatment of 4 courses. The plasma levels of substance P and beta endorphin were compared with the two groups before and after treatment. The VAS scores and PSQI scores and the therapeutic efficacy of the two groups were compared before and after treatment. Results： The results showed that the total effective rate of TCM group was 94. 44%,the total effective rate of the western medicine group was 82. 22%,there was significant difference between the two groups（ P 0. 05）; The two groups after treatment of substance P levels decreased more than before treatment,the Chinese medicine group decreased more（ P0. 05）. After treatment,the two groups of beta endorphin levels before treatment were improved,the traditional Chinese medicine group had greater enhancement（ P 0. 05）; The two groups after treatment VAS score and OS score were lower than before treatment,after treatment of traditional Chinese medicine in the VAS score was lower than that of the western medicine group（ P0. 05）. Conclusion： The treatment of primary trigeminal neuralgia with traditional Chinese medicine decoction can reduce the release of pain mediators,relieve pain,improve sleep,and improve the clinical efficacy. It is worthy of clinical application.
