精神分裂症的中医证候规范化研究

摘　　要：目的探索精神分裂症中医证候辩证规范化标准。方法对湖南省脑科医院纳入诊断的精神分裂症640例患者资料进行统计,结合专家问卷和多因素统计分析等方法得出精神分裂症常见中医证候和相应证候要素,提取各常见证候的关联临床症状及体征。结果纳入的精神分裂症患者640例,频率>5%的中医证候分别为:心脾两虚证74例(11.56%)、阴虚火旺证60例(9.38%)、痰火内扰证59例(9.22%)、肝郁犯脾证58例(9.06%)、脾肾阳虚证55例(8.59%)、痰气郁结证53例(8.28%)和肝胆火旺证43例(6.72%)。上述各中医证候以舌苔和脉象区别各证型之间的区分度良好。结论精神分裂症常见中医证候相关症状及体征的提取工作为进一步精神分裂症中医证候量表的制定提供了客观依据。Objective To explore the traditional Chinese syndrome standardization of schizophrenia. Methods 640 schizophrenia patients diagnosed by Hunan Brain Hospital were investigated. The information collected was statistically analyzed. We aimed to obtain the common TCM （ traditional Chinese medicine ） syndromes, corresponding syndrome elements, TCM syndrome-related clinical symptoms and signs of schizophrenia via expert questionnaire combined with multivariate statistical analysis. Results Among the 640 schizophrenia patients, the common Chinese syndromes （rate 〉 5 % ） are as follows： 74 patients with heart-spleen deficiency syndrome （ 11.56 % ）, 60 patients with yin-deficiency and fire-hyperactivity syndrome （ 9.38 % ）, 59 patients with internal disturbance of pyrophlegm syndrome （ 9.22 % ）, 58 patients with liver qi invading spleen syndrome （ 9.06 c ）, 55 patients with spleen and kid- ney yang deficiency syndrome （ 8.59 % ）, 53 patients with phlegm-qi stagnation syndrome （ 8.28 % ）, and 43 patients with effulgent liver-gallbladder fire syndrome （ 6.72 % ） . Both tongue coat and pulse condition can be well distinguished among the above TCM syndrome. Conclusion The extraction of common TCM syndrome-related clinical symptoms and signs from schizophrenia patients provides an objective evidence for the draft of TCM syndrome scale of schizophrenia.
