摘  要:目的探讨卒中后抑郁(PSD)的脑磁共振低频振幅(ALFF)变化及其与临床症状的关系。方法纳入32例PSD患者及35例年龄、性别、卒中病程相匹配的无PSD脑卒中患者,采用3.0T核磁进行脑部扫描,采用ALFF方法分析两组患者大脑区域活性的变化,并采用Pearson相关性分析差异脑区与蒙哥马利抑郁量表(MADRS)评分的相关性。结果相比对照组,PSD患者存在右侧中央前回(Z=-2.737,P<0.01)、左侧额下回(Z=-2.775,P<0.01)和左侧缘上回(Z=-2.338,P<0.01)的ALFF值降低,且右侧中央前回的ALFF值与MADRS评分呈显著负相关(r=-0.428,P=0.015)。结论 PSD患者大脑存在多个脑区的活性降低,且与临床症状相关,对临床PSD的客观性评估具有重要参考意义。Objective To investigate the changes of cerebral resting amplitude of low frequency fluctuation (ALFF) in patients with post-stroke depression ( PSD ) and its relationship with clinical symptoms. Method 32 patients with PSD and 35 non-PSD stroke patients were enrolled in this study. The ALFF method was used to analyze the changes of brain regions in patients with PSD during in the resting-state functional MRI. Pearson correlation was used to analyze the correlation between different brain regions and the Montgomery Depression Rating Scale ( MADRS ) . Results Compared with the control group, the patients with PSD had decreased ALFF in right precentral gyrus ( Z=-2.737, P〈0.01 ), left inferior frontal gyrus ( Z=-2.775, P〈0.01 ) and left supramarginal gyrus ( Z=-2.338, P〈 0.01 ) . The ALFF of the right precentral frontal gyrus was negatively correlated with the MADRS score (r=-0.428, P = 0.015 ) . Conclusion The activity of multiple brain regions in PSD patients is decreased and correlated with clinical symptoms, which provides an important reference for the objective evaluation of clinical PSD.
