乳癌病人TGF-β1、IL-6和TNF-γ表达及其临床意义

摘  要:目的探讨乳癌病人转化生长因子β1(TGF-β1)、白细胞介素-6(IL-6)和肿瘤坏死因子γ(TNF-γ)的表达及其临床意义。方法选取2015年3月—2016年8月在我院接受治疗的乳癌病人110例作为乳癌组,同时选取110例乳房良性肿瘤病人作为对照组。采用电化学发光法检测两组病人血清TGF-β1、IL-6和TNF-γ的表达。比较不同临床病理特征乳癌病人血清TGF-β1、IL-6和TNF-γ的水平,分析影响乳癌病人血清TGF-β1、IL-6和TNF-γ水平的因素。结果乳癌病人血清TGF-β1水平低于对照组,而IL-6和TNF-γ水平高于对照组,差异均有统计学意义(t=7.539~285.054,P<0.01)。分化程度为中低分化、有淋巴结转移、浸润深度为T3+T4、TNM分期为Ⅲ+Ⅳ期的乳癌病人的TGF-β1水平较低,IL-6和TNF-γ水平较高(t=10.142~50.400,P<0.01),而不同年龄段乳癌病人TGF-β1、IL-6和TNF-γ水平差异无显著性(P>0.05)。以乳癌病人TGF-β1、IL-6和TNF-γ水平作为因变量,进行多重线性逐步回归分析,结果显示分化程度、淋巴结转移和TNM分期进入回归方程。结论乳癌病人TGF-β1表达水平较低,IL-6和TNF-γ表达水平较高,且三者的表达与肿瘤的分化程度、是否存在淋巴结转移和TNM分期密切相关。Objective To investigate the expression and clinical significance of transforming growth factor-beta 1(TGF-β1),interleukin-6(IL-6),and tumor necrosis factor-γ(TNF-γ)in patients with breast cancer.Methods A total of 110 patients with breast cancer who were treated in our hospital from March 2015 to August 2016 were enrolled as breast cancer group,and 110 patients with benign breast tumor were enrolled as control group.Electrochemical luminescence was used to measure the expression of TGF-β1,IL-6,and TNF-γin serum.The serum levels of TGF-β1,IL-6,and TNF-γwere compared between the breast cancer patients with different clinicopathological features,and the influencing factors for serum levels of TGF-β1,IL-6,and TNF-γwere analyzed.Results Compared with the control group,the breast cancer group had a significantly lower serum level of TGF-β1 and significantly higher serum levels of IL-6 and TNF-γ(t=7.539-285.054,P〈0.01).The breast cancer patients with moderately/poorly differentiated tumor,lymph node metastasis,an infiltration depth of T3+T4,and TNM stageⅢ+Ⅳ disease had a lo-wer level of TGF-β1 and higher levels of IL-6 and TNF-γ(t=10.142-50.400,P〈0.01),while there were no significant differences in the levels of TGF-β1,IL-6,and TNF-γbetween the patients in different age groups(P〈0.05).A multiple linear stepwise regression analysis was performed with the levels of TGF-β1,IL-6,and TNF-γin breast cancer patients as dependent variables,and the results showed that degree of tumor differentiation,lymph node metastasis,and TNM stage were included in the regression equation.Conclusion Breast cancer patients have low expression of TGF-β1 and high expression of IL-6 and TNF-γ,and the expression of TGF-β1,IL-6,and TNF-γis closely associated with the degree of tumor differentiation,presence or absence of lymph node metastasis,and TNM stage.
