不同胃黏膜病变组织Survivin和P53蛋白表达及意义

摘　　要：目的探讨Survivin和P53蛋白在不同胃黏膜病变中的表达及意义。方法应用免疫组化方法检测慢性浅表性胃炎、慢性萎缩性胃炎、低级别上皮内瘤变、高级别上皮内瘤变及胃癌组织中Survivin和P53蛋白的表达。结果 Survivin蛋白在慢性浅表性胃炎、慢性萎缩性胃炎、低级别上皮内瘤变、高级别上皮内瘤变、胃癌组织中的阳性表达率分别为0、16.7%、36.7%、46.7%、74.3%,除低级别上皮内瘤变与高级别上皮内瘤变比较差异无显著性外,不同组织Survivin蛋白阳性表达率比较差异均有统计学意义(χ~2=2.06~6.10,P<0.05)。P53蛋白在慢性浅表性胃炎、慢性萎缩性胃炎、低级别上皮内瘤变、高级别上皮内瘤变及胃癌组织中的阳性表达率分别为0、10.0%、36.7%、46.7%、57.1%,除低级别上皮内瘤变与高级别上皮内瘤变比较差异无显著性外,不同组织P53蛋白阳性表达率比较差异均有统计学意义(χ~2=2.00~5.32,P<0.05)。结论 Survivin和P53蛋白表达与胃癌的发生有关,二者的检测对胃癌早期诊断具有重要参考价值。Objective To investigate the expression of Survivin and P53 protein in different types of gastric mucosal lesions and related clinical significance.Methods Immunohistochemistry was used to measure the expression of Survivin and P53 protein in the tissue samples of chronic superficial gastritis,chronic atrophic gastritis,low-grade intraepithelial neoplasia,highgrade intraepithelial neoplasia,and gastric carcinoma.Results The positive rates of Survivin protein in the tissue samples of chronic superficial gastritis,chronic atrophic gastritis,low-grade intraepithelial neoplasia,high-grade intraepithelial neoplasia,and gastric carcinoma were 0,16.7%,36.7%,46.7%,and 74.3%,respectively;there was a significant difference in the positive rate of Survivin protein between any two types of these lesions except between low-and high-grade intraepithelial neoplasia（χ^2=2.06-6.10,P〈0.05）.The positive rates of P53 protein in the tissue samples of chronic superficial gastritis,chronic atrophic gastritis,low-grade intraepithelial neoplasia,high-grade intraepithelial neoplasia,and gastric carcinoma were 0,10.0%,36.7%,46.7%,and57.1%,respectively;there was a significant difference in the positive rate of P53 protein between any two types of these lesions except between low-and high-grade intraepithelial neoplasia（χ^2=2.00-5.32,P〈0.05）.Conclusion The expression of Survivin and P53 protein is associated with the development of gastric carcinoma,and the measurement of Survivin and P53 protein has an important value in the early diagnosis of gastric carcinoma.
