老年消化道肿瘤患者术后营养治疗疗效对比的临床研究

摘　　要：目的探讨术后早期肠内及肠外营养治疗对老年消化道肿瘤患者术后营养状态的改善程度及免疫调节功能的影响。方法采用单中心前瞻随机对照研究,86例老年消化道肿瘤根治术患者按照1∶1随机分为早期肠内肠外营养组(EEN+PN组)和全肠外营养组(TPN组),每组各43例。两组分别于术后第1天开始进行营养治疗,观察术后第7天患者营养指标、免疫功能改善情况,并记录术后恢复情况。结果与TPN组比较,EEN+PN组患者的营养指标(ALB、PA、TF)及免疫功能(Ig A、Ig G、Ig M、CD4+、CD8+、CD4+/CD8+)恢复快且营养治疗费用低,首次排气时间早、住院时间短及术后感染发生率低,上述各项指标的组间差异均有统计学意义(P<0.05)。结论术后早期开展肠内营养联合肠外营养综合治疗对老年消化道肿瘤患者快速康复有非常重要的作用。Objective To investigate the effect of early enteral and parenteral nutrition therapy on nutritional status and immunoregulatory function in senile patients with gastrointestinal cancer. Methods The single center prospective randomized controlled study was employed. A total of 86 elderly patients with gastrointestinal cancer undergoing radical resection were randomly divided into early enteral parenteral nutrition group（ EEN ＋ PN group） and total parenteral nutrition group（ TPN group）,according to 1：1,with43 cases in each group. The two groups were treated with nutritional therapy on the first day after operation respectively. The nutritional indicators and immune function improvement of patients on the seventh day after operation were observed,and postoperative recovery was recorded. Results Compared with TPN group,the nutritional index and immune function of patients in EEN ＋ PN group recovered quickly,and the cost of nutrition treatment was low. The time of first exhaust was earlier,and the incidence of postoperative infection was low. There were statistical difference in the above indexes were dtetcted between the two groups（ P〈0. 05）. Conclusion Early enteral nutrition combined with parenteral nutrition therapy has an important role in the rapid recovery of postoperative elderly patients with gastrointestinal tumor.
