摘 要:目的:探讨头颅MRl对亚低温治疗新生儿缺氧缺血性脑病远期预后的诊断价值。方法:通过计算机检索维普中文科技期刊数据库(VIP)、中国期刊全文数据库(CNKI)、万方数据库、中国生物医学文献数据库(CBM)、Embase、Cochrane图书馆、Pubmed中(从建库至2017年7月)获得头颅MRI对亚低温治疗新生儿缺氧缺血性脑病远期预后文献,用MetaDisc1.4软件检验各文献间的异质性,并根据异质性结果选择相应的效应模型,计算敏感度和特异度及其95%CI,RevMan5.3软件绘制森林图。结果:建立外文文献库235篇,中文文献库34篇,选出8篇符合纳入标准。Meta分析的结果显示MR/的汇总敏感度和特异度分别为0,78(95%C10.70-0.84),0.76(95%C10.70-0.82)。早期MRI的汇总敏感度和特异度分别为1(95%C10.84~1)、0.66(95%C10.49-0.80)。晚期MRI的汇总敏感度和特异度分别为0.81(95%C10.54-0.96),0.64(95%C10.50-0.76)。结论:MR/对亚低温治疗新生儿缺氧缺血性脑病远期预后的特异度及敏感度均较高,是目前评价新生儿缺氧缺血性脑病远期预后的较好方法。Objective: To systematically the prognostic value of MRI in neonatal patients with hypoxic ischemie eneephalopathy (HIE) treated with hypothermia. Methods: A search Jn VIP, China National Knowledge Infrastructure (CNKI), Wanfang Chinese Periodical Database and Chinese Bio -medicine Database (CBM), Embase, Cochrane library, Pubmed was performed to identity retrieve relevant studies on MRI value of hypoxic iscbemic encephalopathy treated with hypothermia. ( From its establishment to July 2017 ). Statistical analysis was performed by employing MetaDisc 1.4 . Heterogeneity of the included articles was tested for selecting proper effect model to calculate pooled weighted sensitivity, specificity and 95% CI. Results:We searched 235 relevant English articles, 34 relevant Chinese articles. Eight studies were met the inclusion criteria. Sensitivity of test for diagnostic of MR] was 78 (95 % CI 0.70 0.84) %, specificity was 76% (95 % CI 0. 70 0.82) o The pooled sensitivity and specificity were 1 (95 0. 84 1,0.66 CI% (95) % CI 0.49 0.80 ) for Early MR], The pooled sensitivity and specificity were 0.81 (95 0.54 0.96, 0.64 CI% (95) % CI 0.50 0.76 ) for late. Conclusion: MRI has high specificity and sensitivity for the long term prognosis of hypoxic ischemic encephalopathy treated by mild hypothermia. Therefore, MR] is the best way to evaluate the long term prognosis of mild hypothermia treatment of neonatal hypoxic ischemic eucephalopathy.
