西藏山南地区内镜活检HER2阳性胃癌临床病理特征分析

摘　　要：目的分析西藏山南地区内镜活检胃癌HER2阳性率,探讨HER2阳性胃癌临床病理特征。方法收集胃癌活检标本84例,采用免疫组织化学法(IHC)检测HER2蛋白表达,对IHC(++)病例进一步采用荧光原位杂交(FISH)检测HER2基因状态。结果 84例胃癌组织HER2 IHC阳性率为11.9%(10/84),IHC联合FISH检测HER2阳性率为17.9%(15/84)。Lauren分型肠型胃癌HER2阳性率高于混合型/弥漫型(χ~2=5.843,P<0.05)。高/中分化胃癌HER2阳性率高于低分化胃癌(χ~2=6.321,P<0.05)。HER2阳性率在不同年龄、性别、民族、肿瘤部位、Hp感染情况中无统计学差异(P>0.05)。结论胃癌HER2阳性与Lauren分型及组织学分化程度密切相关。Objective To investigate the positive rate of HER2 in gastric cancer and the clinicopathologic characteristics of HER2 positive gastric cancer by endoscopic biopsy. Methods HER2 protein of 84 gastric cancer simples was examined by immunohistochemistry（ IHC）. Then,HER2 gene was detected in IHC（ ＋ ＋） cases by fluorescence in situ hybridization（ FISH）. Results HER2 positive rate was 11. 9%（ 10/84） in gastric cancer cases by IHC method and 17. 9%（ 15/84） by IHC combined FISH method. HER2 positive rate in intestinal type gastric carcinoma was significantly higher than that in mixed and diffuse types（ χ^2= 5. 843,P〈0. 05）. HER2 positive rate in poorly differentiated type was lower than that in well and moderately differentiated gastric cancer（ χ^2= 6. 321,P〈0. 05）. There was no statistical difference in HER2 positive rates among gastric cancer subgroups stratified by age,gender,race,tumor location,Hp infection（ P〉0. 05）. Conclusion HER2 positive is associated with Lauren classification and WHO classification/differentiation in gastric cancer.
