摘  要:目的:了解2型糖尿病(Type 2 diabetes mellitus,T2DM)患者恶性肿瘤的患病情况,并分析T2DM合并恶性肿瘤患者代谢因素与肿瘤临床特征的相关性。方法:连续收集2012年1月至2016年12月笔者单位收治的6 202例T2DM患者的临床资料,进一步按照是否并发恶性肿瘤(恶性肿瘤在T2DM之后或同时确诊)分为病例组和对照组,计算T2DM患者恶性肿瘤的患病率,采用多因素Logistic回归模型分析T2DM合并恶性肿瘤患者代谢相关因素与肿瘤临床特征的相关关系。结果:在6 202例新发T2DM患者中,有468例合并有恶性肿瘤,患病率为7.55%;男性患病率8.47%高于女性患病率6.66%,差异有统计学意义(χ2=7.241,P=0.007);T2DM恶性肿瘤组患者的平均年龄明显高于T2DM组[(59.21±10.77)岁vs.(56.18±13.74)岁,t=4.621,P<0.001]。多因素Logistic回归分析显示:糖尿病病程、Hb A1c与T2DM合并恶性肿瘤患病存在显著的正相关关系,是T2DM合并恶性肿瘤的独立危险因素(OR值分别为4.383,3.603,均P<0.05);TG与T2DM合并恶性肿瘤存在显著的负相关关系,是T2DM合并恶性肿瘤的保护因素(OR值为0.861,P<0.05)。结论:T2DM患者合并恶性肿瘤的患病率较高,糖尿病病程、Hb A1c是T2DM合并恶性肿瘤的独立危险因素,TG是其保护因素。Objective: To investigate the prevalence of malignant tumor in type 2 diabetes mellitus patients,and analyze the correlation between metabolic status and clinical characteristics. Methods: The clinical data of 6 202 cases of T2DM patients were analyzed,and divided into the case group and the control group according to whether or not with malignant tumor,to calculate the prevalence of malignant tumor in T2DM patients. Multivariate Logistic regression model was used to analyze the correlation between metabolic status and clinical characteristics for malignant tumor in T2DM patients.Results: In 6 202 new cases of T2DM patients,468 cases with malignant tumor,the prevalence was 7. 55%; the prevalence of male was higher than female( 8. 47% vs. 6. 66%),the difference was statistically significant( χ2= 7. 241,P =0. 007); the average age of malignant tumor in T2DM patients were significantly higher than those of the T2DM group[( 59. 21 ± 10. 77) vs.( 56. 18 ± 13. 74),t = 4. 621,P 0. 001]. Multivariate Logistic regression analysis showed that Hb A1 c and T2DM of diabetes,patients with malignant tumors was significant positive correlation between the prevalence of T2DM,and is an independent risk factor associated with malignant tumors( OR = 4. 383,3. 603,P 0. 05); TG and T2DM associated with malignant tumors have negative correlation significantly,is a protective factor for T2DM associated with malignant tumors( OR = 0. 861,P 0. 05). Conclusion: T2DM patients with a higher prevalence of malignant tumors,diabetes,HbA1c is an independent risk factor for T2DM associated with malignancy,TG is the protective factor.
