摘　　要：目的:探讨子宫内膜癌患者血清人附睾分泌蛋白4(human epididymal secretory protein 4,HE4)、糖类抗原125(carbohydrate antigen 125,CA125)水平变化及其临床价值。方法:选取我院126例子宫内膜癌患者作为恶性组,同期81例子宫内膜增生患者作为良性组,114例体检健康女性作为对照组,观察各组HE4、CA125变化情况,用ROC曲线对HE4、CA125子宫内膜癌最佳诊断切点实施分析,评价两种指标单独检验与联合测定诊断子宫内膜癌的准确性及临床价值。结果:HE4诊断子宫内膜癌相比CA125具有明显优势(P<0.05);各组患者HE4、CA125水平对比,差异具有统计学意义(P<0.05);恶性组HE4、CA125水平显著高于良性组及对照组(P<0.05)。HE4诊断子宫内膜癌灵敏度、特异度、符合率等均高于CA125,单独检验差异不具有统计学意义(P>0.05)。结论:联合测定HE4、CA125这两种指标诊断子宫内膜癌具有较高灵敏度与特异度,HE4诊断子宫内膜癌效果优于CA125,HE4与CA125联合检验具有互补作用,可以避免单独检验发生的误诊漏诊,提高临床诊断子宫内膜癌准确率。Objective： To investigate the levels of serum human epididymal secretory protein 4（ HE4） and carbohydrate antigen 125（ CA125） in patients with endometrial carcinoma and their clinical value. Methods： There were126 cases of patients with endometrial cancer as the malignant group,81 cases of endometrial hyperplasia patients as benign group,114 healthy women as control group. HE4,CA125 were measured by ROC curve analysis,and single and combined detection of HE4,CA125 were compared. Results： HE4 has an obvious advantage over endometrial cancer than the diagnosis of CA125（ P＜0. 05）. HE4,CA125 levels of the patients in each group were compared,and the difference was statistically significant（ P＜0. 05）. The levels of HE4 and CA125 in malignant group were significantly higher than those in benign group and control group（ P＜0. 05）. The sensitivity,specificity and coincidence rate of endometrial carcinoma diagnosed by HE4 were higher than those of CA125,and the difference was not statistically significant（ P＜0. 05）. Conclusion： The combined detection of HE4 and CA125 in the diagnosis of endometrial cancer has higher sensitivity and specificity. HE4 in diagnosis of endometrial carcinoma is better than CA125. HE4 and CA125 joint inspection has complementary effect,can avoid the occurrence of separate inspection misdiagnosis,improve the accuracy of clinical diagnosis of endometrial carcinoma.
