摘 要:目的分析As_2O_3联合全反式维甲酸治疗成分输血后儿童急性白血病的疗效及对免疫蛋白的调控作用,为临床治疗提供参考。方法以本院2013年10月~2017年10月期间诊治的成分输血后160名急性白血病儿童为研究对象,其中60例接受As_2O_3联合全反式维甲酸治疗,50例接受As_2O_3单药治疗,50例接受全反式维甲酸单药治疗。分析比较三组患儿的临床疗效,检测治疗前后患儿免疫蛋白与细胞凋亡蛋白水平的变化。结果与As_2O_3或全反式维甲酸单药组相比,As_2O_3联合全反式维甲酸治疗的患儿早期死亡率更低(P<0.05),CR比例更高(P<0.05),到达CR的时间更短(P<0.05),且IgA、IgM、Caspase3及Caspase9蛋白水平升高更显著(P<0.05)。结论 As_2O_3联合全反式维甲酸对成分输血后儿童急性白血病有良好效果,且治疗后免疫蛋白及细胞凋亡蛋白水平明显升高。Objective To explore the effect and its regulation of immunoglobulins of As2O3 combined with all-trans retinoic acid in the treatment of acute leukemia children after transfusion. Methods 160 children with acute leukemia after transfusion were enrolled in this study in our hospital between Oct 2013 and Oct 2017. The effect of As2O3 combined with all-trans retinoic acid in the treatment of acute leukemia children after transfusion and its regulation of immunoglobulin were assayed. Acute leukemia children in the control group were given As2O3 or all-trans retinoic acid. Results As2O3 combined with all-trans retinoic acid showed better effect on acute leukemia children than As2O3 or all-trans retinoic acid alone with statistical differences (P〈0.05). And the concentrations of IgA, IgM, Caspase3, and Cas-pase9 were decreased greatly after As2O3 combined with all-trans retinoic acid treatment (P〈0.05). Conclusion As2O3 combined with all-trans retinoic acid is effective for the treatment of acute leukemia children after transfusion and can regulate immunoglobulin concentration.
