频谱边界频率与意识障碍患者临床预后的相关性研究

摘  要:目的:观察频谱边界频率与缺血性脑卒中、出血性脑卒中及心肺复苏后缺血缺氧性脑病意识障碍患者预后的相关性。方法:对缺血性脑卒中、出血性脑卒中及缺血缺氧性脑病致意识障碍患者分为嗜睡、昏睡、浅昏迷、中昏迷、深昏迷5组,分析发病24 h内95%频谱边界频率与GCS及意识障碍程度的相关性,并分析95%频谱边界频率与缺血性脑卒中组、出血性脑卒中组及缺血缺氧性脑病组3组患者发病1个月后预后的相关性。结果:(1)3组疾病的患者95%频谱边界频率与GCS评分均呈正相关,与意识障碍程度呈负相关。(2)95%频谱边界频率与出血性脑卒中及缺血缺氧性脑病发病1个月后的良好预后呈正相关,与缺血性脑卒预后无明显相关性。结论:95%频谱边界频率可客观、正确地评估患者意识障碍程度,并可预判出血性脑卒中及心肺复苏后缺血缺氧性脑病的短期预后,但对缺血性脑卒中预后无明显预判作用。Objective To analyze the correlation between spectral edge frequency and clinical prognosis in disturbance of consciousness patients with ischemic cerebral apoplexy,hemorrhagic cerebral apoplexy and hypoxic-ischemic encephalopathy after cardiopulmonary resuscitation(CPR). Method The patients with ischemic cerebral apoplexy,hemorrhagic cerebral apoplexy and hypoxic-ischemic encephalopathy after CPR were divided into 5 groups: somnolence,sopor,shallow coma,middle coma,deep coma. Analyse of the correlations between 95% SEF and GCS,95% SEF and disturbance of consciousness degree within 24 hours after onset. And analyse of the correlations between 95% SEF and clinical prognosis in disturbance of consciousness patients with ischemic cerebral apoplexy,hemorrhagic cerebral apoplexy and hypoxic-ischemic encephalopathy after 30 Days. Results(1)There were positive correlation between 95% SEF and GCS,and negative correlation between 95% SEF and the degree of disturbance of consciousness for all patients.(2)There were positive correlation between 95% SEF and clinical prognosis in disturbance of consciousness patients with hemorrhagic cerebral apoplexy and hypoxic-ischemic encephalopathy after CPR,and negative correlation between 95% SEF and clinical prognosis in disturbance of consciousness patients with ischemic cerebral apoplexy. Conclusion 95% SEF can evaluate the degree of disturbance of consciousness,and can predict the clinical prognosis in disturbance of consciousness patients with hemorrhagic cerebral apoplexy and hypoxic-ischemic encephalopathy after CPR,not ischemic cerebral apoplexy.
