不同碎核技术在过熟期白内障超声乳化术中的应用

摘　　要：目的观察不同碎核技术在过熟期白内障超声乳化术中的应用效果。方法回顾性临床研究。收集2017年4~8月期间我院过熟期白内障超声乳化术患者30例(30只眼),术中采用裸核旋转雕刻法碎核者为观察组12例(12只眼);术中采用常规拦截劈核法碎核者为对照组18例(18只眼);分析比较两组手术后视力及术中术后并发症情况。结果两组术后视力较术前均有明显提高,术后1 d、3 d、1周、1个月视力比较两组差异无统计学意义;术中对照组5例后囊膜破裂,均发生在乳化核的过程中,观察组无后囊破裂,两组比较有统计学差异(χ2=3.91,P<0.05),两组均无明显角膜水肿和眼压升高情况。结论过熟期白内障超声乳化术中应用裸核旋转雕刻法碎核可以恢复视力,减少术中后囊膜破裂的发生率,提高手术安全性。Objective To observe the effects of various nucleofractis techniques in hypermature phacoemulsification Methods This study was a retrospective clinical study. 30 patients with hypermature cataracts were divided into two groups. All patients underwent phacoemulsification between April and August in 2017. In 12 eyes rotary engraving nucleofractis techniques was used（ observation group） and in 18 eyes stop-and-chop techniques（ control group） was used. Postoperative visual acuity and complications were analyzed and compared. Results Postoperative visual acuity improved in both groups and there was no significant difference between the groups on 1 day,3 days,1 week,or 1 month. The rate of posterior capsule rupture was significantly different（ χ2= 3. 91,P 0. 05）. No apparent corneal edema or increased intraocular pressure were found. Conclusions Application of rotary engraving nucleofractis techniques in hypermature cataract phacoemulsification is safe and effective. It improves postoperative visual acuity and reduces the rate of posterior capsule rupture.
