老年腱膜性上睑下垂的个性化手术治疗

摘　　要：目的探讨老年腱膜性上睑下垂的个性化手术治疗方法,并评价其临床效果。方法对15例(27只眼)老年上睑下垂,依据上睑下垂程度、提上睑肌肌力及术中观察到的提上睑肌腱病变情况,分别行提上睑肌腱膜修复、折叠或缩短术,联合美容性上睑皮肤松弛矫正术,术后随访3~12个月。结果除2例(4只眼)重度下垂,术中发现提上睑肌腱膜脂肪样变性者,欠矫1~2 mm,其余均较满意,上睑下垂基本矫正,达到治疗和美容的双重效果。结论采用个性化提上睑肌腱膜修复术联合上睑皮肤松弛矫正术治疗老年腱膜性上睑下垂,安全有效。Objective To explore individualized surgical treatment for senile ptosis and evaluate its clinical effect.Methods Recovering,folding or excision of the upper lid levator muscle aponeurosis was chosen for 15 patients with senile ptosis（ n = 27 eyes）. Selection of the surgery type was based on degree of ptosis,degree of levator muscle weakness,and severity of aponeurosis damage. The follow-up period was 3 ~ 12 months. Results Most patients obtained satisfactory improvement in symptoms as well favorable cosmetic effects,except in 2 cases（ n = 4 eyes） of severe ptosis that fatty degeneration of lacertus muscle aponeurosis was found and the ptosis was undercorrected by 1-2 mm. Conclusions Individualized surgery is safe and effective to treat senile ptosis.
