摘  要:目的检测妊娠期糖尿病(GDM)患者血清中胰高血糖素样肽-1(GLP-1)的水平,探讨GLP-1在GDM发生发展过程中的临床意义。方法选取2013-2015年于天津市中心妇产科医院就诊的孕期妇女共200例,行口服葡萄糖糖耐量试验(OGTT)及胰岛素释放试验,分为糖耐量正常(NGT)组80例,妊娠期糖尿病(GDM)组及糖耐量受损(IGT)组各60例。分别检测各组OGTT各时间点(0、1、2 h)血糖、胰岛素及GLP-1水平。结果与NGT组比较,GDM组3个时间点血糖水平均升高,GLP-1水平均下降,差异有统计学意义(P<0.05);0 h与2 h胰岛素分泌增多,差异有统计学意义(P<0.05),且峰值延迟,2 h左右达到分泌最高峰。IGT组3个时间点血糖较NGT组升高,差异有统计学意义(P<0.05);GLP-1水平变化差异无统计学意义(P>0.05);2 h胰岛素分泌增多,差异有统计学意义(P<0.05),分泌峰值未出现延迟。结论 GDM患者存在GLP-1分泌水平下降,其可能参与了胰岛素抵抗和胰岛β细胞功能受损的进程,对于它的检测和评估可能作为GDM诊疗的一个潜在的新型靶点。Objective To detect the glucagon like Peptide -1 (GLP-1) levels in serum of patients with gestational diabetes mellitus (GDM), and investigate its clinical significance in the development of the disease. Methods 200 pregnant women from Tianjin Central Obstetrics and Gynecology Hospital during 2013 to 2015 were carried out the oral glucose tolerance test (OGTF) and insulin release test and divided into three groups: the group of normal glucose tolerance (NGT) with 80 cases, the group of gestational diabetes mellitus (GDM) with 60 cases and the group of impair glucose tolerance (IGT) with 60 cases. The glucose, the insulin and the GLP-1 in each time point of OGTT (0, 1, 2 h) were measured and compared between three groups. Results Compared with the NGT group, the levels of glucose in three time points were all significantly increased and the GLP-1 levels were all significantly decreased in GDM group, the differences were statistically significant (P〈0.05). At the time points of 0 h and 2 h, the level'of insulin were significantly increased, the differences were statistically significant (P〈0.05). The secretion at 2 h was the highest, and the peak value was delayed to release. The levels of glucose in three time points in IGT group were higher than those in NGT group, the differences were statistically significant (P〈0.05), and there were no significant differences in the changes of GLP-1 levels(P〉0.05). The secretion of insulin at 2 h was increased, the difference was statistically significant (P〈0.05) and the peak value was not delayed to release. Conclusion Patients with GDM have the lack of GLP-1 secretion, it may involved in the development of insulin resistance and decline in pancreatic β-cell function. The detection and evaluation of GLP-1 may be a potential target for the diagnosis and treatment of GDM.
