摘 要:目的:观察功能性电刺激(FES)联合任务导向性训练对痉挛型脑瘫儿童上肢运动功能和ADL的影响。方法:选取痉挛型脑瘫儿童60例,随机分为任务导向性训练组(n=30)和FES+任务导向性训练组(n=30),任务导向性训练组采用伸手取物、前臂旋转、对指运动、对指+前臂旋转训练和家庭训练,FES+任务导向性训练组在任务导向性训练及家庭训练的基础上,同时应用FES训练。两组儿童分别于治疗前、治疗8周、12周后后,采用Peabody运动发育量表(PDMS)中的抓握项目、精细运动能力评定表(FMFM)及改良Barthel指数(MBI)分别对精细运动功能和日常生活活动能力进行评估和比较分析。结果:8周康复治疗后,两组儿童与治疗前评分比较差异均具有显著性(P<0.05),而FES+任务导向性训练组评分明显高于任务导向性训练组,差异具有显著性(P<0.05);12周康复治疗后与治疗前比较两组儿童差异更具显著性(P<0.01),而FES+任务导向性训练组评分明显高于任务导向性训练组,差异具有显著性(P<0.01)。结论:FES联合任务导向性的长期训练可有效提高痉挛型脑瘫儿童上肢运动功能和ADL能力。Objective: To observe the effect of ADL and function of upper limb of children with spastic cerebral palsy who were trained with Functional Electrical Stimulation( FES) united task oriented training. Methods:Sixty children with spastic cerebral palsy were randomly divided into task oriented training group( n = 30) and FES + task oriented training( n = 30). Task oriented training group received training including reaching for things,forearm rotation,finger-to-finger action,finger-to-finger action + forearm rotation,and family training. On the basis of task oriented training and family training,FES + task-oriented training received FES training as well. The two groups were evaluated by values of Grasping Item of Peabody Developmental Motor Scale( PDMS),Fine Motor Function Measure( FMFM) and Modified Barthel Index( MBI) before the treatment,8 weeks after treatment and 12 weeks after treatment. Results: After 8 weeks treatment,the scores of PDMS,FMFM and MBI were significantly different between the two groups compared with the scores before treatment( P〈0. 05); and the score of FES + task oriented training was significantly higher than that of task oriented training( P〈0. 05). After 12 weeks treatment,the scores of PDMS,FMFM and MBI were significantly different between the two groups compared with the scores before treatment( P〈0. 01); and the score of FES + task oriented training was significantly higher than that of task oriented training( P〈0. 01). Conclusion: FES + task oriented training can effectively improve the upper limb function and ADL of children with spastic cerebral palsy.
