老年2型糖尿病患者血糖水平对种植牙状态的影响

摘　　要：目的探讨老年2型糖尿病患者的血糖水平对种植牙状态的影响。方法收集口腔种植修复老年2型糖尿病患者120例均分为两组。术后5年内,FBG≥7.0mmol/L和/或HbA1c≥7.0%的患者为观察组(60例),FBG<7.0mmol/L和/或HbA1c<7.0%为对照组(60例)。比较两组术后即刻(T1)和术后1(T2)、3(T3)、6(T4)个月及术后1(T5)、5(T6)年时种植牙稳定性参数(ISQ值)、种植体边缘骨吸收量值和种植体周围炎发病情况。结果 T1、T2时,两组ISQ值和种植体边缘骨吸收量值比较均无统计学差异(P>0.05)。T3~T6时,观察组ISQ值低于对照组,而种植体边缘骨吸收量值高于对照组(P<0.01)。对照组较观察组种植体周围炎发病颗数少[10%(6/60)vs.40%(24/60)](P<0.05)。结论老年2型糖尿病患者的血糖水平对种植牙的近期影响不明显,但血糖水平升高对种植牙状态的远期影响突出,术后应积极控制血糖水平。Objective To observe the effect of blood glucose（BG） level on dental implant status in elderly patients with type 2 diabetes mellitus（T2DM）. Methods According to BO levels in 5 years after operatrion, 120 elderly T2DM patients were divided into two groups of A（FBG〈7.0 mmol/L and/or HbA1c%7.0% ,60 cases） and B（FBG≥7.0 mmol/L and/or HbA1c≥7.0%, 60 cases）. The dental stability parameter（ISQ value）, marginal bone resorption and implant periqmplantitis were observed immediately after operation（T1） and in the 1st （T2）, 3rd （T3）, 6th （T4） month and 1st （TS） and 5th （T6） year after operation. Results At T1 and T2, there was no significant difference in ISQ value and marginal bone resorption between two groups（P〉0.05）. At T3-T6, the ISQ of group B was lower than that of group A, while the marginal bone resorption of group B was higher than that of group A（P〈0. 01）. The percentage of the patients with implant peri-implantitis in group A was lower than that in group B[10%（6/60） vs. 40%（24/60）]（P〈0. 05）. Conclusion The BG level of elderly T2DM patients has no obvious effect on the dental implants in the short-term, but higher BG level in the long-term effects obviously on the dental implant status. Attention should be paid to the BG level after operation in T2DM patients underwent dental implant.
