摘 要:目的比较眼球解剖学指标中央角膜厚度(central corneal thickness,CCT)、眼轴长度(axial length,AL)及角膜屈光力(corneal curvature,CCV)在原发性青光眼及正常人之间有无差异。分析CCT、AL及CCV三者之间的关系。方法横断面研究。对284人行CCT、AL及CCV检查。其中正常眼压性青光眼(normal tension glaucoma,NTG)60例(60眼),原发性开角型青光眼(primary open angle glaucoma,POAG)75例(75眼),原发性闭角型青光眼(primary angle closure glaucoma,PACG)89例(89眼)及正常人60例(60眼)。对四组的结果进行比较分析。结果 NTG,POAG,PACG及正常人组CCT分别为528.58±35.11μm,544.96±35.92μm,537.44±31.74μm,539.80±31.88μm;AL分别为24.24±1.41mm,24.30±1.61mm,22.47±0.99mm,23.73±1.38mm;CCV分别为44.82±1.75,44.96±1.78.45.04±1.62,44.75±1.46。NTG较POAG组CCT薄,P=0.005,其余各组之间CCT无差异(P>0.05)。PACG组AL短于其他3组(P<0.05),POAG和NTG组AL较正常人略长(P<0.05)。四组间CCV差异无统计学意义(P=0.781)。各组内CCT与AL、CCV均不相关(P>0.05),AL均与CCV呈负相关。结论不同类型青光眼患者在解剖学指标中的CCT和AL上存在差异,CCV无差异,但CCV与AL存在相关性。上述差异除AL与闭角型青光眼发病之间的关系较为明确之外,其他结果是否与青光眼发病存在关系尚不明确,尤其是薄角膜是否为青光眼的独立危险因素,仍需进一步研究。Objective To investigate the anatomic index of eyeball including central corneal thickness (CCT) , axial length (AL) and corneal curvature (CCV) in primary glaucoma patients and normal controls, and to explore the relationship among CCT, AL and CCV. Methods This was a cross-sectional study. 284 subjects (284 eyes) were enrolled , including 75 cases of primary open angle glaucoma (POAG) , 60 cases of normal tension glaucoma (NTG) , 89 cases of primary angle closure glaucoma (PACG) and 60 cases of normal controls. Parameters including CCT, CCV and AL were analysis. Results The mean CCT was 528.58 +135.11 μm, 544.96±35.92 μm, 537.44±31.74 μm, 539.80± 31.88 μm in NTG, POAG, PACG and normal controls, the mean AL was 24.24±1.41mm, 24.30±1.61mm, 22.47± 0.99 mm, 23.73±1.38mm and the mean CCV was 44.82±1.75,44.96±1.78, 45.04±1.62, 44.75±1.46 respectively . The difference in CCT between POAG and NTG was significant (P =0.005). The other groups showed no significance in CCT (P 〉0.05 ). The AL was smaller in PACG than in other groups(P 〈0.05 ). POAG and NTG had longer AL than normal controls . CCV showed no significance in all groups. The correlation betweenALandCCVwerer=-0538, P〈0.001 (NTG) ; r =- 0.339, P=0.004 (POAG) ; r=-0.710, P〈0.001 (PACG) ; r=-0.379, P=0.003 (normal). Conclusion In patients with different types of glaucoma, CCT and AL showed some differences while no significant difference in CCV. CCV and AL in all groups showed negative correlation. In addition to the clear relationship between AL and the incidence of PACG, the relationship between the other results and the incidence of glaucoma is not clear. Fur- ther research is still needed.
