单胎与多胎早产儿视网膜病变发病率的比较

摘　　要：目的研究并分析单胎与多胎早产儿视网膜病变(Retinopathy of prematurity,ROP)发病率差异。方法分析2010年5月至2017年3月昆明市儿童医院眼科就诊的单胎与多胎早产儿及足月小样儿(出生体重≤2000g)共1791例,使用宽视野数字化眼底照相机(Retcamll)进行视网膜检查,收集单胎和多胎ROP发病数据出生体重、孕周、胎数、ROP病情分期、吸氧时间、住院时间等,并进行X^2检验和t检验统计分析。结果 1791例早产儿中发现ROP317例,ROP发生率为17.70%。接受筛查的早产儿中,单胎1211例,ROP发生率为16.35%;多胎580例,ROP发生率为20.52%;两组间比较差异有统计学意义(X^2=4.675,P=0.031)。在所有ROP病例中,严重病变(≥3期)共59例,发生率为18.61%,单胎ROP严重病变发生率为18.69%,多胎ROP严重病变发生率为18.49%;两组间比较差异无统计学意义(X^2=20.000,P=0.998)。结论在昆明地区,ROP发生率为17.70%;多胎妊娠的早产儿ROP发生率较高,但严重程度与单胎没有差别。Objective To determine differences in incidence of retinopathy of prematurity between multiple- gestation and single-gestation and to analyze differences in severity of ROP among multiple-gestation and single- gestation in Kunming. Methods 1791 cases of prematurity and infants with birth weight was equal or less than 2000g were examined by Retcam II and birth weight, gestational age, number of gestation, stage of ROP, oxygen time, time in hospital were recorded. （ May 2010-March 2017）. Datas were analyzed using X2 test and t test. Results 1211 singles with 198 ROP cases （16.35%）; 580 multiples with 119 ROP cases （20.52%）. The comparison between two groups was statistically significant. In this study, among all ROP patients, severe ROP （≥3 stage） is 18.61%, and the incidence of serious pathology among singles is 18.69 %, multiples is 18.49%, no statistical significance between the two groups. Conclusions In Kunming, the incidence of ROP is 17.70%. The incidence of ROP in multiples is higher than singles, but serious pathology is not different .
