“原络配穴”与补肾益寿胶囊治疗脑梗死伴排尿障碍(肾虚证)的临床研究

摘　　要：目的观察原络配穴与补肾益寿胶囊对脑梗死伴排尿障碍(肾虚证)患者血清同型半胱氨酸水平、智能损害、排尿障碍及中医证候评分的影响,并探讨其机制。方法将脑梗死肢体活动障碍伴排尿障碍患者120例按随机数字表法分为针刺治疗组、补肾益寿胶囊组、针刺联合组3组,在控制原发病等常规治疗与护理基础上,针刺治疗组以原络配穴为配穴原则,予针刺治疗;补肾益寿胶囊组予补肾益寿胶囊0.6 g口服,每日3次;针药联合组予原络配穴联合口服补肾益寿胶囊,观察3组治疗前后血清同型半胱氨酸水平、长谷川痴呆量表(HDS)评分、排尿障碍(包括B超查残余尿、尿液动力学改变)及中医证候评分。结果 3组治疗后同型半胱氨酸水平、HDS评分、B超查残余尿、尿液动力学、中医证候评分均明显改善(P<0.01);针刺治疗组与补肾益寿胶囊组差异无统计学意义(P>0.05),针药联合组各项指标改善程度优于针刺治疗组与补肾益寿胶囊组(P<0.01)。结论原络配穴与补肾益寿胶囊联合用于脑梗死患者可明显改善血清同型半胱氨酸水平、长谷川痴呆量表(HDS)评分、排尿障碍及中医证候评分。Objective： To observe the effect of combined selection of Yuan point and Luo point and Bushen Yishou Capsule on serum homocysteine level ,intelligent damage ,dysuria and TCM syndrome score in patients with cerebral infarction and urinary incontinence. Methods： 120 patients with cerebral infarction and limb dys- plasia were randomly divided into 3 groups according to the random number table method. On the basis of routine treatment and nursing care ,the acupuncture treatment group followed the principle of combined selection of Yuan point and Luo point with acupuncture treatment. Bushen Yishou Capsule were given 0.6 grams of Bushen Yishou Capsule orally,3 times a day;the combined group was treated with combined selection of Yuan point and Luo point combined with oral Bushen Yishou Capstde. Before and after treatment,serum homocysteine levels, Hasegawa dementia score （HDS）, dysuria （including B-ultrasonography residual urine, urinary dynamic changes） and TCM syndrome scores were observed in three groups. Results： The levels of homocysteine, Hasegawa demen- tia scale （HDS）,B-uhrasonography residual urine,urinary kinetics and TCM score we, re significantly improved in the three groups （P 〈 0.01 ）. There was no significant difference between the acupuncture treatment group and Bushen Yishou Capsule group （P〉 0.05）. The improvement of the indexes in the combined group was better than that in the acupuncture group and the drug group （P〈 0.01 ）. Conclusion： The combined selection of Yuan point and Luo point combined with Bushen Yishou Capsule can improve the serum homocysteine level,the Hasegawa Dementia Scale（HDS） score, the urinary obstruction and the TCM syndrome score.
