玻璃体内注射雷珠单抗治疗视网膜静脉阻塞黄斑水肿的效果

摘　　要：目的评估玻璃体内注射雷珠单抗治疗视网膜静脉阻塞（RVO）继发黄斑水肿的效果。方法2016年3月至2017年7月RVO继发黄斑水肿接受玻璃体内雷珠单抗注射术44例（44眼）。使用频域OCT技术（SD-OCT）测量术前及术后1、3、及6个月的中心凹下脉络膜厚度及黄斑中心区视网膜厚度。分别评估玻璃体内注射雷珠单抗后视网膜厚度以及脉络膜厚度的改变及相关因素：年龄、高血压病史、吸烟、饮酒、阻塞部位、发病时间、术前脉络膜厚度及术前视网膜厚度。结果注射后1、3、及6个月中心凹下脉络膜厚度黄斑中心区视网膜厚度均较术前减小（均为P〈0．05）。中心凹下脉络膜厚度改变量仅与其注射前的厚度相关，黄斑部视网膜厚度改变量与注射前的厚度、阻塞部位及发病时间相关。结论玻璃体内注射雷珠单抗治疗RVO黄斑水肿效果良好。Objective To evaluate the efficacy of intravitreal injection of Ranibizumab for the treatment of macular edema secondary to retinal vein occlusion （RVO）. Methods Forty-four eyes of 44 cases with macular edema secondary to RVO from Mar. 2016 to Jul. 2017 were collected and they received intravitreal injection of Ranibizumab. The subfoveal choroidal thickness and central macular retinal thickness were examined by SD-OCT before injection and 1, 3, 6 month after injection. The change of subfoveal choroidal thickness and central macular retinal thickness after injection were evaluated, and the factors related to thickness change ： age, history of hypertension, smoking, drinking, site of occlusion, time of onset and preoperative conditions were evaluated. Results The subfoveal choroidal thickness and the central retinal thickness 1,3, 6 months after injection were less than that before surgery （ all P 〈 0.05 ）. The change of the subfoveal choroidal thickness was related to the subfoveal choroidal thickness preoperative. The change of the central retinal thickness was related to the central retinal preoperative, site of occlusion and the disease time. Conclusion The intravitreal injection of Ranibizumab is effective for the macular edema secondary to RVO.
