气压治疗联合低分子肝素预防高血压脑出血术后深静脉血栓的临床评价

摘　　要：目的探讨间歇性充气加压治疗联合低分子肝素预防高血压脑出血术后瘫痪下肢深静脉血栓的安全性与有效性。方法 100例高血压脑出血术后患者随机被分为对照组、实验组各50例,两组患者均行常规治疗、护理,实验组在此基础上采用间歇性充气加压联合低分子肝素钙治疗。在治疗前及治疗后第1d、7d、14d行下肢血管彩超检查、头颅ct检查。结果实验组深静脉血栓发生率低于对照组,实验组平均血流流速高于对照组,有统计学差异(P<0.05)。实验组未发生皮肤破溃、坏死、出血等不良事件。结论间歇性充气加压联合低分子肝素钙能安全、有效地预防高血压脑出血术后患者瘫痪肢体深静脉血栓。Objective to evaluate safety and effectiveness of intermittent pneumatic compression combined with low molecular weight heparin on prophylaxis of deep venous thrombosis of paralyzed lower limb in patients after hypertensive intracerebral hemorrhage surgery. Methods 100 postoperative patients were randomly divided into control group and experimental group,50 cases in each group, two groups were performed route therapy and care,experimental group was received therapy of intermittent pneumatic compression and low molecular weight heparin in addition. color doppler and ct scan were performed on lower limb before and at 1 st day、7 th day、14 th day after treatment. Results The incidence of deep venous thrombosis was lower and the average blood flow velocity was higher in experimental group than those in the control group,there was statistical significance（P0.05）. There was no skin break, necrosis and bleeding in experimental group. Conclusion intermittent pneumatic compression combined with low molecular weight heparin therapy is safe and effective in prevention of deep venous thrombosis of paralyzed limb in patients after hypertensive intracerebral hemorrhage surgery.
