摘　　要：目的 探讨经胼胝体-穹窿间入路切除第三脑室内肿瘤对穹窿结构的影响.方法 回顾性分析2015年12月至2016年6月清华大学玉泉医院神经外科收治的5例第三脑室内肿瘤患者的临床资料.5例患者均经胼胝体-穹窿间入路切除肿瘤.分别于术前,术后1、2、3个月行弥散张量成像（DTI）检查,分析穹窿各向异性分数（FA）、表观弥散系数（ADC）以及穹窿长度的变化情况.结果 5例患者均行肿瘤全切除.术后1个月,3例患者穹窿的FA值稍有下降,1例ADC值稍有下降,4例穹窿长度缩短.至术后3个月,4例患者穹窿的ADC值,3例患者穹窿的FA值、长度呈现向术前状态恢复的趋势.结论 经胼胝体-穹窿间入路手术对第三脑室内肿瘤患者穹窿结构的影响不明显,且多可恢复.DTI功能分析可作为评估手术安全性的有效手段. Objective To investigate the fornix of patients undergoing resection of third ventricle tumor through transcallosal interforniceal approach.Methods The clinical data of 5 patients with third ventricle tumor were retrospectively analyzed who were admitted to Department of Neurosurgery,Tsinghua University Yuquan Hospital from December 2015 to June 2016.All patients received operation through transcallosal interforniceal approach.DTI （diffusion tensor imaging） was performed before and after operation.The fractional anisotropy （FA）,apparent diffusion coefficient （ADC） values and length of fornix were measured.Results Total resection was achieved in 5 cases.At 1 month post operation,the DTI functional analysis showed that the FA value of fornix decreased slightly in 3 patients,the ADC value decreased slightly in 1 patient and the length of fornix decreased in 4 patients,compared with those before operation.Those parameters returned to preoperative levels at 3 months after operation.Conclusions The effects of operation on fornix were slight and temporary.After proficient operation,there was no significant change in the value of FA,ADC or the length of the fornix.DTI could be an effective method to evaluate the safety of operation.
