摘 要:目的:探讨针刺对皮质扩散抑制(CSD)偏头痛大鼠内源性镇痛系统的影响。方法:将50只SD大鼠随机分为空白对照组(不予任何处理)、模型对照组(只使用氯化钾刺激大鼠皮层诱发CSD)、针刺预防组(建模前连续针刺大鼠太冲和阳陵泉穴10 d,建模后不再针刺)、针刺治疗组(建模成功后针刺大鼠太冲和阳陵泉穴3周)、针刺防治组(建模前连续针刺大鼠太冲和阳陵泉穴10 d;建模成功后继续进行针刺治疗3周)。采用放射免疫分析法检测针刺前后大鼠血浆中降钙素基因相关肽(CGRP)、β-内啡肽(β-EP)、5-羟色胺(5-HT)含量变化,并利用免疫组化SABC法对针刺后中脑导水管周围灰质(PAG)区CGRP、β-EP、5-HT、c-fos的含量进行测定并比较。结果:(1)利用氯化钾溶液刺激大鼠枕叶皮层可成功建立CSD偏头痛模型。(2)模型对照组与空白对照组比较,存在着行为学改变,血浆CGRP含量升高、β-EP和5-HT含量降低(均P<0.01),脑干PAG区CGRP和c-fos阳性细胞数升高、β-EP和5-HT阳性细胞数降低(均P<0.01)。(3)针刺预防组、针刺治疗组、针刺防治组与模型对照组比较,行为学改变持续时间缩短,血浆CGRP含量降低、β-EP和5-HT含量升高(均P<0.01),脑干PAG区CGRP和c-fos阳性细胞数降低、β-EP和5-HT阳性细胞数升高(P<0.05或P<0.01)。(4)与针刺预防组和针刺治疗组相比,针刺防治组大鼠的行为学改变持续时间减少更明显,血浆β-EP含量更低,CGRP和5-HT含量更高(P<0.05或P<0.01),脑干PAG区CGRP和c-fos阳性细胞数更低、β-EP和5-HT阳性细胞数更高(P<0.05或P<0.01)。结论:针刺可通过影响CSD偏头痛内源性镇痛系统有效防治偏头痛。Objective:To investigate the effects of acupuncture on the endogenous analgesic system in migraine rats with cortical spreading depression(CSD). Methods:Fifty SD rats were randomly divided into five groups:blank control group(without any treatment);model control group(only with potassium chloride stimulating rat cortex to induce CSD);acupuncture prevention group(rats were treated with acupuncture at Taichong and Yanglingquan acupoints for 10 d continuously before being modeled but no longer with acupuncture after modeling);acupuncture treatment group(rats were treated with acupuncture at Taichong and Yanglingquan acupoints after being modeled for 3 weeks);acupuncture control group(rats were treated with acupuncture at Taichong and Yanglingquan acupoints for 10 d continuously before being modeled,and with successive acupuncture after modeling for 3 weeks). Radioimmunoassay was used to detect the changes of calcitonin gene-related peptide(CGRP),β-EP,and 5-HT in rat plasma before and after acupuncture. The contents of CGRP,β-EP,5-HT and c-fos in the periaqueductal gray(PAG)area of the mid-brain after acupuncture were determined by immunohistochemical SABC method. Then the results were compared. Results(1) The model of migraine rats with CSD were successfully established by stimulation of potassium chloride on occipital cortex of rats.(2) Compared with the blank control group,the model control group had behavioral changes,increased plasma CGRP levels,decreased β-EP and 5-HT levels(P〈0.01). The number of CGRP and c-fos positive cells was increased,and the number of β-EP and 5-HT positive cells was decreased in PAG area of the brainstem in the model control group compared with that in the blank control group(P〈0.01).(3) Compared with the model control group,the acupuncture prevention group,acupuncture treatment group,and acupuncture control group had shorter duration of behavior change,lower plasma CGRP content,and increased β-EP and 5-HT levels(P〈0
