利培酮对青年女性精神分裂症患者催乳素、血清瘦素及糖脂代谢的影响研究

摘　　要：目的：探讨利培酮对青年女性精神分裂症患者催乳素、瘦素及糖脂代谢的影响。方法：选择78例于本院就诊的青年女性精神分裂症患者，采取随机数字表法设计分为利培酮组与奥氮平组。采取PANSS法对两组治疗前、治疗后1、2、4、8周的疗效予以评估，比较两组治疗前后催乳素、瘦素、胆固醇、空腹血糖、体质指数等糖脂代谢指标。结果：两组显效及总有效率比较，统计学分析未见明显差异性（P〉0．05）；8周后PANSS各指标显示好转，差异具有统计学意义（P〈0．05）；8周后，奥氮平组患者PANSS阳性症状低于对照组（P〈0．05），但其余各时间段比较，两组PANSS无统计学差异（P〉0．05）。奥氮平组除空腹血糖其余各项糖脂代谢指标上升水平明显高于利培酮组，且均高于治疗前水平，统计学分析差异性明显（P〈0．05）；利培酮组催乳素、瘦素水平低于奥氮平组（P〈0．05）。结论：利培酮治疗青年女性精神分裂症患者，疗效与奥氮平具有同等疗效，对于患者催乳素、瘦素、糖脂代谢的影响小于奥氮平，具有推广应用价值。Objective： The study aims to investigate the effects ofrisperidone on prolactin, leptin and glucose and lipid metabolism in young female schizophrenics. Methods： We selected 78 young female schizophrenics in our hospital. The patients were divided into risperidone group and olanzapine group by random number table method. PANSS method was used to evaluate the curative effect of two groups before, 1, 2, 4 and 8 weeks after treatment. The indexes of glucose and lipid metabolism, such as prolactin, leptin, cholesterol, fasting blood glucose and body mass index, were compared between the two groups before and after treatment. Results： There is no significant difference （P〉0.05） between the two group analysis and efficiency comparison. The PANSS index showed improvement after 8 weeks, and the difference was significant （P〈0.05） . After 8 weeks, olanzapine group of patients with PANSS positive symptoms was lower than those in the control group （P〈0.05） , but the rest of the time, there was no significant differences between the two groups under PANSS （P〉0.05） . The increase in glucose and lipid metabolism in olanzapine group levels were significantly higher than the risperidone group, except for fasting blood sugar, and were higher than the level before treatment. The statistical analysis was significantly different （P〈0.05） ; The prolactin and leptin levels were lower than that in risperidone olanzapine group （P〈0.05） . Conclusion： Risperidone in the treatment of young female patients with schizophrenia has the same curative effect as olanzapine, and has less influence on prolactin, leptin and glycolipid metabolism than olanzapine, and has the value of popularization and application.
