轻型复发性阿弗他溃疡患者治疗前后唾液皮质醇和分泌型免疫球蛋白A的相关性研究

摘　　要：目的:探讨轻型复发性阿弗他溃疡患者治疗前后唾液皮质醇和分泌型免疫球蛋白A水平的变化和临床意义。方法:使用医院焦虑抑郁量表记录焦虑、抑郁量表得分。应用酶联免疫吸附法对50名轻型复发性阿弗他溃疡患者治疗前后唾液中唾液皮质醇和SIg A水平进行测定,并与30名健康人群作对比,记录检测结果并分析焦虑、抑郁得分与唾液皮质醇和SIg A水平的相关性。结果:轻型复发性阿弗他溃疡患者治疗后同治疗前相比,焦虑量表得分和抑郁量表得分均降低(P<0.01),唾液皮质醇水平显著降低(P<0.01),SIg A水平显著升高(P<0.01)。治疗前唾液皮质醇水平与焦虑量表和抑郁量表得分呈正相关(r=0.575,0.529,P<0.01),SIg A水平与焦虑量表和抑郁量表得分呈负相关(r=-0.653、-0.551,P<0.01)。结论:轻型复发性阿弗他溃疡的发生发展与焦虑抑郁等心理因素有关。Objective： To investigate changes of salivary cortisol and Secretory immunoglobulin A before and after treatment in patients suffering from minor aphthous ulcer. Methods： Using the Hospital Anxiety and Depression Scale to record the anxiety and depression score. The method of enzyme linked immunosorbent assay was used to detect the levels of salivary cortisol and Secretory immunoglobulin A in 50 minor aphthous ulcer patients before and after treatment. And compared with 30 healthy people. Record test results and analysis the correlation between Anxiety and Depression score and the level of salivary cortisol and Secretory immunoglobulin A. Results： The patients in minor aphthous ulcer group after treatmen,the scores of Anxiety and Depression and the level of salivary cortisol were significantly lower than the patients before treatmen（P 0. 01）,while the level of Secretory immunoglobulin A was significantly higher than the patients before treatmen（P 0. 01）. The level of salivary cortisol in patients after treatment were positively correlated with the scores of Anxiety and Depression（r = 0. 575,0. 529,P 0. 01）. The level of Secretory immunoglobulin A in patients after treatment were negatively correlated with the scores of Anxiety and Depression（r =-0. 653、-0. 551,P 0. 01）. Conclusion： The pathogenesis and exacerbation of minor aphthous ulcer could be inferred by psychological factor.
