局部灌注治疗重症急性胰腺炎感染临床疗效分析

摘　　要：目的探讨局部灌注治疗重症急性胰腺炎(severe acute pancreatitis,SAP)感染的临床疗效。方法我院ICU接受治疗的SAP感染患者82例,按照随机数字表法分为局部灌注组和全身组各41例。全身组行常规治疗+药物全身治疗,局部灌注组予常规治疗+局部灌注治疗。观察两组血糖、血淀粉酶(AMY)、谷丙转氨酶(ALT)、血肌酐(Scr)、胰高血糖素;血清C反应蛋白(CRP)、肿瘤坏死因子(TNF-α)、血清白介素-6(IL-6)、白介素-1β(IL-1β)等炎性因子;血清CD3^+、CD4^+、CD8^+、CD4^+/CD8^+等T细胞水平。结果治疗后局部灌注组血糖、AMY、ALT、Scr、胰高血糖素水平,血清IL-6、IL-1β、TNF-α、CRP、CD8^+水平均低于全身组,血清CD3^+、CD4^+、CD4^+/CD8^+水平均高于全身组,总有效率高于全身组(P<0.05)。结论局部灌注治疗SAP感染可有效改善肝肾功能,清除炎性因子,纠正免疫失衡,疗效显著。Objective To investigate the clinical effect of local perfusion therapy in the treatment of patients with severe acute pancreatitis（ SAP） infection.Methods A total of 82 patients with SAP infection treated in ICU of our hospital were randomly divided into local perfusion group（ n = 41） and general group（ n = 41）.The general group received routine treatment plus drug general treatment while the local perfusion group received routine treatment plus local perfusion therapy. The blood glucose,blood amylase（ AMY）,alanine aminotransferase（ ALT）,serum creatinine（ Scr）,glucagon,inflammatory factors such as serum C reactive protein（ CRP）,tumor necrosis factor-α（ TNF-α）,serum interleukin-6（ IL-6） and interleukin-1β（ IL-1β）,and T lymphocytes such as CD3^＋,CD4^＋,CD8^＋and CD4^＋/CD8^＋were observed in the both groups.Results After treatment,the levels of blood glucose,AMY,ALT,Scr and glucagon,serum levels of IL-6,IL-1β,TNF-α and CRP and lymphocyte CD8^＋in the local perfusion group were all lower than those in the general group.In contrast,levels of CD3^＋,CD4^＋and CD4^＋/CD8^＋in the local perfusion group were higher than those in general group（ P〈0. 05）.The total effective rate of the local perfusion group was higher than that in the general group（ P〈0. 05）. Conclusion Local perfusion therapy in the treatment of SAP infection can effectively improve the hepatic and renal function,eliminate the inflammatory factors and correct the immune imbalance.The curative effect is significant.
