替吉奥(S-1)诱导及同步放化疗治疗老年局部晚期鼻咽癌患者近期疗效观察

摘　　要：目的观察老年鼻咽癌局部进展期患者采用替吉奥诱导化疗并同步放化疗的疗效及副反应。方法我院就诊的老年鼻咽癌患者30例,分期Ⅲ~Ⅳa,采用替吉奥60 mg/m^2,D-14诱导化疗两周期后再以同样方案同步放化疗,统计及分析治疗的疗效及副反应。结果所有患者均完成诱导化疗及放疗,其中有6例患者未完成2个周期的同步化疗。所有患者均达到了完全缓解或部分缓解,其中完全缓解率56.7%(17/30),部分缓解43.3%(13/30)。不良反应主要表现为恶心、呕吐、腹泻及口腔黏膜炎,除1例患者发生4级口腔黏膜炎外未观察到其余的4级副反应。结论替吉奥诱导化疗并同步放化疗治疗局部晚期老年鼻咽癌疗效肯定,副反应不大,值得临床推广应用。Objective To observe the clinical effects and side effects of S-1 as a single agent for induction chemotherapy and concurrent chemo-radiotherapy in the treatment of elder patients with locally advanced nasopharyngeal carcinoma.Methods Thirty elder patients with locally advanced nasopharyngeal carcinoma（ III-Iva stage） treated in our hospital were enrolled in this clinical observation.60 mg/m^2 S-1 was used as induction chemotherapy for 2 cycles（ one cycle = 14 days） followed by a concurrent chemo-radiotherapy.The clinical effects and side effects were statistically analyzed.Results Majority patients had completed two cycles＇ induction chemotherapy and chemo-radiotherapy,but 6 patients have not completed 2 cycles concurrent chemotherapy with S-1. All patients achieved complete remission（ CR） or partial remission（ PR）,in which 17 patients achieved CR（ 56. 7%） and 13 patients achieved PR（ 43. 3%）.The side effects mainly presented as nausea,vomit,diarrhea and oral mucositis.No grade 4 side effects were observed except one patient who suffered from a grade 4 oral mucositis.Conclusion S-1 as single agent in induction and concurrent chemo-radiotherapy in the treatment of elder patients with locally advanced nasopharyngeal has affirmative clinical effects and moderate side effects. It is worthy of promotion.
