摘 要:目的探讨鼻咽癌(NPC)靶区勾画学习曲线的规律。方法选取2016年7月至2017年5月在郑州大学第一附属医院放疗科接受放射治疗的35例NPC患者,按手术时间先后顺序分为7组(A^G组),每组5例,每例均进行完整的靶区勾画,比较每组的性别、年龄、体质量指数(BMI)、KPS评分、平均勾画时间、危及器官靶区精确度、放疗靶区精确度和急性放疗毒性反应等指标。结果 7组患者在年龄、性别、BMI、KPS评分和急性放疗毒性反应、危及器官精确度方面比较,差异无统计学意义(P>0.05)。7组勾画时间及靶区精确度比较,差异有统计学意义(P<0.05)。A、B、C组勾画时间比较,差异有统计学意义(P<0.05);A、B、C组和D、E、F、G组之间比较,差异有统计学意义(P<0.05);D、E、F、G组之间比较,差异无统计学意义(P>0.05)。A、B、C、D组之间GTV和CTV靶区精确度比较,差异有统计学意义(P<0.05);A、B、C、D组和E、F、G组之间比较,差异有统计学意义(P<0.05);E、F、G组之间比较,差异无统计学意义(P>0.05)。结论对于已熟练肿瘤放射治疗学理论知识的年轻放疗医生,经过20例鼻咽癌靶区后,可望顺利、安全渡过该学习曲线。Objective To explore the learning curve of the delineation of radiotherapy target area for nasopharyngeal carcinoma. Methods A total of 35 patients with nasopharyngeal carcinoma who underwent radiotherapy from July of 2016 to May of 2017 were retrospectively analyzed. All the patients were divided into seven groups( A-G) according to the radiotherapy date,5 cases in each group. The age,sex,body mass index( BMI),KPS score,average time,accuracy of clinical target volumes and organs at risk,acute radiation toxicity were compared among seven groups. Results There was no significant difference in age,sex,BMI,KPS score,accuracy of organs at risk and acute radiation toxicity among the seven groups( P 0. 05). There were significant differences in average time and accuracy of organs at risk among seven groups( P 0. 05). There were significant differences in average time among groups A,B,C( P 0. 05). There were significant differences in average time among groups A,B,C and D,E,F,G( P 0. 05). There was no significant difference among groups D,E,F,G( P 0. 05). There were significant differences of accuracy of CTV and GTV among seven groups( P 0. 05). There were significant difference among groups A,B,C,D and E,F,G( P 0. 05). There was no significant difference among groups E,F and G( P 0. 05). Conclusion New radiation therapists who have mastered the skilled knowledge of radiotherapy can make themselves master of the accurate determination of target volume in nasopharyngeal carcinoma after the practice of 20 cases.
