摘 要:目的探讨常规超声联合弹性成像对非哺乳期乳腺癌的诊断价值分析。方法选择非哺乳期乳腺癌患者98例,以及同期在该医院就诊的非哺乳期乳腺炎患者43例。所有患者均行常规超声(US)以及超声弹性成像(UE)检查,结合病理学检查结果比较US及UE诊断非哺乳期乳腺疾病的影像学结果,分析其检出率,同时评价两种检查手段对非哺乳期乳腺疾病的诊断效能。结果经病理组织检查非哺乳期乳腺炎43例患者共检出52个病灶,非哺乳期乳腺癌98例患者共检出158个病灶;US检测通过边缘参数分辨非哺乳期乳腺癌和乳腺炎的检出率差异无统计学意义(P>0.05),US其他参数,包括是否出现微小钙化、病灶后方是否有回声,病灶内部血流等级等分辨非哺乳期乳腺炎和乳腺癌的检出率差异有显著差异(P<0.05);UE两个参数弹性成像评分以及RI等分辨非哺乳期乳腺炎和乳腺癌的检出率差异显著(P<0.05);UE诊断非哺乳期乳腺炎和乳腺癌的敏感性、特异性以及阳性预测值、阴性预测值均高于US;UE联合US检出非哺乳期乳腺炎和乳腺癌的敏感性、特异性以及阳性预测值、阴性预测值高于单一超声检测。结论常规超声联合超声弹性成像用于诊断非哺乳期乳腺癌的诊断价值较高,可提高诊断的准确性,为临床医师确诊病情提供有效依据。Objective To explore the diagnostic value of conventional ultrasound combined with ultrasonic elstography for non-lactating breast cancer. Methods 98 patients with non-lactating breast cancer were collected,and 43 patients with nonlactating mammitis were enrolled in the same period. All patients were examined with conventional ultrasound( US) and ultrasonic elastography( UE),and the results were compared and analyzed,detection rate combined with pathological examination was used to evaluate the diagnostic efficacy for non-lactating breast disease. Results Through pathological examination,52 lesions were detected in 43 patients with non-lactation mastitis,and 158 lesions were detected in 98 patients with non-lactating breast cancer;The difference of detection rate with US detection via edge parameters to distinguish non-lactation mastitis and breast cancer was no significant( P〉0. 05),and the difference with other US parameters,including micro calcification or not,echo in the posterior part of the lesion or not,the blood flow level et al,to distinguish mastitis and breast cancer was significant( P〈0. 05);UE score and RI to distinguish the difference for the detection rate of non lactation mastitis and breast cancer were significantly( P〈0. 05);The diagnostic sensitivity,specificity and positive predictive,negative predictive value of UE to distinguish non lactation mastitis and breast cancer were higher than those US;The diagnostic sensitivity,specificity and positive predictive,negative predictive value of UE combined with US to distinguish non lactation mastitis and breast cancer were higher than those single ultrasonic testing. Conclusion The diagnostic value of UE combined with US for non lactation breast cancer is high,and which could improve the accuracy of diagnosis and provide effective evidence for the diagnosis of the disease.
