无痛胃镜下套扎术对肝硬化食管静脉曲张治疗的安全性研究

摘　　要：目的:通过对无痛胃镜与普通胃镜下食管静脉曲张套扎术(EVL)的安全性比较,探讨无痛胃镜下套扎术的优势。方法:选择2015年10月-2017年9月在笔者所在医院住院确诊的肝硬化食管静脉曲张患者共92例,将其随机分为普通胃镜组(n=45)和无痛胃镜组(n=47),行胃镜下食管静脉曲张套扎术,观察术中呕吐、呛咳等不良反应发生情况及术前5 min、术中、术后5 min的心率、收缩压、舒张压和血氧饱和度变化。结果:无痛胃镜组套扎术完成率高于普通胃镜组,不良反应发生率低于普通胃镜组,内镜操作时间短于普通胃镜组,比较差异均有统计学意义(P<0.05);无痛胃镜组术中心率较术前5 min稍减慢,普通胃镜组术中心率较术前5 min稍增快,但比较差异均无统计学意义(P>0.05);普通胃镜组术中心率高于无痛胃镜组,比较差异有统计学意义(P<0.05);无痛胃镜组术中收缩压、舒张压较术前5 min均降低,而普通胃镜组术中较术前5 min收缩压、舒张压均升高,且普通胃镜组术中收缩压、舒张压均高于无痛胃镜组,比较差异均有统计学意义(P<0.05);两组血氧饱和度在术前5 min、术中和术后5 min均在正常范围且无明显变化,比较差异均无统计学意义(P>0.05)。结论:无痛胃镜下套扎术对肝硬化食管静脉曲张患者的治疗安全性高,较普通胃镜更有优势。Objective：To compare the safety of painless gastroscopy and ordinary gastroscopy in endoscopic variceal ligation（EVL）,and to explore the advantages of EVL under painless gastroscopy.Method：From October 2015 to September 2017,a total of 92 patients hospitalized and diagnosed with cirrhosis and esophageal varices in the author＇s hospital were selected and randomly divided into ordinary gastroscopy group（n=45） and painless gastroscopy group（n=47）. EVL was performed.The occurrence of adverse reactions such as vomiting and coughing during surgery and the changes of heart rate（HR）,systolic blood pressure（SBP）,diastolic blood pressure（DBP） and oxygen saturation（SpO_2） at 5 minutes before surgery,during surgery,5 minutes after surgery were observed.Result：The rate of completion of ligation of painless gastroscopy group was higher than that of ordinary gastroscopy group,the incidence of adverse reactions was lower than that of ordinary gastroscopy group,and the endoscopic operation time was shorter than that of ordinary gastroscopy group,the differences were statistically significant（P〈0.05）.The HR in the painless gastroscopy group was slightly slower than that of 5 minutes before surgery,the HR in ordinary gastroscopy group was slightly faster than the 5 minutes before surgery,but the differences were not statistically significant（P〈0.05）.The intraoperative HR in the ordinary gastroscopy group was higher than that in the painless gastroscopy group,and the difference was statistically significant（P〈0.05）.In the painless gastroscopy group,the intraoperative SBP and DBP were lower than those of 5 min before surgery,but in the ordinary gastroscopy group,the intraoperative SBP and DBP were higher than those of 5 min before surgery,and the intraoperative SBP and DBP were higher in the ordinary gastroscopy group than those in the painless gastroscopy group,the differences were statistically significant（P〈0.05）.There was no significant difference in SpO_2 between the gr
