安氏Ⅱ类1分类错牙合畸形患者功能性矫治的疗效分析

摘　　要：目的:检测Twin-Block矫治器对安氏Ⅱ类1分类错牙合畸形患者的疗效,为临床治疗提供依据。方法:选择30例安氏Ⅱ1类错牙合畸形患者,采用Twin-Block矫治器行早期矫治,采用Ricketts头影测量分析法测量主要标志点和参考平面。结果:与矫治前比较,矫治后患者下颌体长度(Xi-Pm)增加2.50 mm(P<0.05),SNB角增加2.58°(P<0.05),ANB角平均减少2.71°(P<0.01);磨牙关系距平均减小2.81mm(P<0.01),前牙覆盖平均减小3.68mm(P<0.01),下中切牙垂距(L1-OP)减小1.89mm(P<0.01),下中切牙突度距(distance of L1-APog)增加1.84mm(P<0.01),上中切牙倾斜角(U1-APog)减小7.35°(P<0.01);下唇突度(Ls-TVL)增加1.85 mm(P<0.05),颏沟深度(Sm-TVL)增加2.12 mm(P<0.05),颏突度(Pos-TVL)增加2.89mm(P<0.05)。结论:应用Twin-Block矫治器矫治处于生长发育期的安氏Ⅱ类1分类错牙合畸形患者能有效促进其下颌骨生长发育,内收上前牙,改善上前牙唇倾度,改善侧貌。Objective：To detect the clinical effect of Twin-Block appliance in the treatment of the patients with ClassⅡ division 1 malocclusion,and to provide the basis for clinical treatment.Methods：A total of 30 patients with classⅡ division 1 malocclusion were treated with Twin-Block appliance.The variables of the patients before and after treatment were measured with Ricketts cephalometric radiographys.Results：Compared with before treatment,the length of corpora mandibulae（Xi-Pm）of the patients after treatment was increased by 2.50 mm（P〈0.05）,the SNB angle was increased by 2.58°（P〈0.05）,and the ANB was decreased by 2.71°（P〈0.01）;the molar relationship distance and the Overjet were decreased by 2.81 mm and 3.68 mm,respectively（P〈0.01）;the L1-OP was decreased by 1.89 mm,the distance of L1-APog was increased by 1.84 mm,and the angle U1-APog was decreased by 7.35°（P〈0.01）;the Ls-TVL was increased by 1.85 mm,the Sm-TVL was increased by 2.12 mm（P〈0.05）,and the Pos-TVL was increased by 2.89 mm（P〈0.05）.Conclusion：The Twin-Block appliance could obviously promote the development of mandibular bone,upright the anterior teeth and improve the lip angle of upper anterior teeth and the proflie in the treatment of the patients with classⅡ division 1 malocclusion in the growth development period.
