平阳霉素碘油乳剂栓塞治疗肝海绵状血管瘤对肝功能的影响

摘　　要：目的探讨平阳霉素碘油乳剂栓塞治疗肝海绵状血管瘤对肝功能的影响。方法选取2010-03至2016-12医院收治且随访资料完全的肝海绵状血管瘤患者62例,均行平阳霉素碘油乳剂栓塞治疗,分别于术前、术后3 d及术后14 d检测血液中丙氨酸氨基转移酶(alanine amiotransferase,ALT)、天门冬氨酸氨基转移酶(aspartate aminotransferase,AST)、γ-谷氨酰转移酶(y-glutamyl transpeptadase,GGT)、碱性磷酸酶(alkaline phosphatase,ALP)、血清总胆红素(total bilirubin,TBIL)、直接胆红素(direct bilirubin,DBIL)、间接胆红素(indirect bilirubin,IBIL)等指标,并对检测结果进行统计学分析。结果术后3 d,TBIL为(16.33±6.66)μmol/L,DBIL为(3.63±2.08)μmol/L,ALT为(80.09±82.63)U/L,AST值为(40.31±19.46)U/L,均较术前增高,差异有统计学意义(P<0.05);术后14 d再次复查肝功能指标,ALT为(16.14±6.38)U/L,AST为(17.97±4.54)U/L,TBIL为(12.17±4.68)μmol/L,DBIL为(2.13±1.09)μmol/L,与术前相比差异无统计学意义。GGT、ALP、IBIL等指标治疗前后没有统计学差异。结论平阳霉素碘化油乳剂栓塞治疗肝血管瘤在短时间内会引起肝功能相关指标增高,一般在14 d内即可恢复正常,无严重胆道毁损并发症发生。Objective To investigate the influence of Pingyang mycin-lipiodol emulsion on liver function in patients with hepatic cavernous hemangioma. Methods Sixty-two hepatic cavernous hemangioma patients who had complete follow-up data were treated with interventional therapy in our hospital between March 2010 and December 2016. The changes of test results regarding alanine aminotransferase(ALT）,aspartate aminotransferase(AST）,γ-glutamyl transferase(GGT）,alkaline phosphatase(ALP）,total bilirubin(TBIL）,direct bilirubin(DBIL） and indirect bilirubin(IBIL） were retrospectively analyzed with statistical methods before interventional embolization and three days and two weeks after embolization. Results The levels of TBIL,DBIL,ALT and AST were separately(16. 33 ± 6. 66） μmol/L,(3. 63 ± 2. 08） μmol/L,(80. 09 ± 82. 63） U/L and(40. 31 ± 19. 46） U/L three days after embolization. There was significant difference(P 〈 0. 05）. After two weeks,ALT,AST,TBIL,DBIL and IBIL were(16. 14 ± 6. 38） U/L,(17. 97 ± 4. 54 U/L）,(12. 17 ± 4. 68） μmol/L and(2. 13 ± 1. 09） μmol/L respectively,and the difference was not statistically significant when compared to the preoperative results. GGT,ALP and IBIL did not change significantly after treatment. Conclusions Embolization with Pingyang mycin-lipiodol emulsion in hepatic cavernous hemangioma patients can result in transient increase of ALT,AST,TBIL and DBIL. Liver function tests can return to normal usually within two weeks. There are no serious complications of biliary duct damage.
