安氏Ⅱ~2分类错畸形拔牙矫治前后软硬组织变化的临床研究

摘　　要：目的探讨安氏Ⅱ~2分类错牙合畸形患者拔牙矫治前后软硬组织的变化。方法选取30例拔除上颌第一双尖牙,下颌第一或第二双尖牙并以直丝弓矫治技术完成的安氏Ⅱ~2分类、骨性Ⅱ类凸面型错牙合患者,应用X线头影测量技术比较矫治前后软硬组织的变化。结果安氏Ⅱ~2分类错牙合畸形患者拔牙矫治前后多项软硬组织指标差异存在统计学意义(P<0.05),其中SNA角、ANB角、A'Ls-FH角变小,SNB角、U1-NA距离、U1-NA角、N-Me、ANS-Me、FH-Ns-Pg'、Ns-Sn-Pg'、Z角变大。结论安氏Ⅱ~2分类、骨性Ⅱ类凸面型错牙合畸形拔牙矫治后,软硬组织得到明显改善,使患者面型更接近正常面型。Objective To study the changes of soft and hard tissue before and after orthodontic treatment in ClassⅡ division 2 malocclusion with extraction. Methods 30 Class Ⅱ division 2, skeletal class Ⅱ convex shape of the face malocclusion adult with extraction of the maxillary first premolar and mandibular first or second premolar were treated by straight wire appliance with extractions. The pre-treatment and post-treatment cephalograms were digitized,and several cephalometric variables were examined to evaluated the changes of soft and hard tissue. Results A number of soft and hard tissues were significantly changed compared with measurements before treatment in Class Ⅱ division 2 malocclusion（P 〈0.05）. The SNA angle, the ANB angle, A’Ls-FH anglewas significantly decreased after the treatment. The SNB angle, The line distance between U1-NA、the angle between U1-NA、N-Me、ANS-Me、FH-NsPg’、Ns-Sn-Pg’, Z angle was significantly increased after the treatment. Conclusion Soft and hard tissue after orthodontic treatment in ClassⅡ division 2,skeletal class Ⅱ convex shape of the face malocclusion with extraction was improved,and soft tissue profile will be more esthetical.
