舌前腺的形态学研究及临床应用

摘　　要：目的研究舌前腺的解剖形态学以改进舌前腺囊肿的手术方式。方法研究标本来源于尸体解剖和外科手术,观察舌前腺的解剖学形态和组织学形态,采用常规手术方法和改进手术方法治疗舌前腺囊肿,比较术后反应及愈后情况。结果 (1)舌前腺呈近纺棰体形,长8~10mm、宽3~6.5mm、厚3~6mm,位于舌尖腹部舌系带两侧,由舌肌包裹无明显包膜。(2)舌前腺是以粘液腺泡为主的混合性腺体,腺泡团块呈分叶状,内有细小的分泌导管和粗细不一的排泄导管。(3)舌前腺由舌神经支配,舌动脉供血,舌腹静脉回流。(4)改进手术方法治疗舌前腺囊肿的治愈率95.65,常规手术方法的治愈率为93.33,改进手术方法复发率与传统手术方法复发率之间没有明显差异(P>0.05)。改进手术方法术后3天的肿胀情况明显低于常规手术方法(P<0.01),7天后的肿胀情况无明显差异。结论改进手术方法是舌前腺囊肿治疗和防止复发以及减轻术后反应的有效手术方式。Objective To study the anatomical morphology of the anterior lingual salivary gland to improve the surgical approach of the anterior lingual salivary gland cyst. Methods The study specimens were derived from autopsy and surgical operation. The anatomical morphology and histological morphology of the anterior lingual salivary glands were observed. The conventional surgical method and the modified surgical method were used to treat the anterior lingual salivary gland cyst, and the postoperative response and prognosis were compared. Results（1） Anterior lingual salivary gland was nearly spindle shape, length 8-10 mm, width 3-6.5 mm, thickness 3-6 mm, located in the tongue side of the tongue on both sides of the tongue, wrapped by the tongue muscle no capsule obviously.（2）Anterior lingual salivary gland is mucus acinar-based mixed glands, acellular mass was lobulated, with a small secretory duct and different thickness of the drainage catheter.（3） The anterior lingual salivary gland supplyed by the lingual nerve, tongue artery blood, tongue and abdomen reflux.（4）Improve the cure rate of the treatment of anterior lingual salivary gland cysts was 95.7%, the cure rate of conventional surgical methods was 93.3%, there is no significant difference between the two methods（P〉0.05）,the swelling after 3 days in mproved surgical procedureswas significantly lower than the conventional surgical method（P〈0.01）, the swelling after 7 days was no significant defference（P〉0.05）. Conclusion Improved surgical approach is an effective surgical approach for the treatment of cyst and the prevention of recurrence and the reduction of postoperative response.
