摘 要:目的针对下颌磨牙游离缺失的病例,应用三维有限元法探讨天然牙-种植体联合固定桥在不同的种植位点和天然牙数目下的应力分布情况。方法应用CT扫描技术与Mimics、UG、Ansys软件相结合,建立左下颌天然牙-种植体联合双端固定桥和单端固定桥及其支持组织的三维有限元模型,分别在分散垂直加载和斜向加载下,研究固定桥中种植体及其周围骨组织的应力分布情况。结果天然牙-种植体联合固定桥中种植体骨界面Von Mises应力均集中于种植体颈部骨皮质处;斜向载荷下各模型Von Mises应力明显提高;增加天然牙数目使TISBs中种植体颈部的应力值减小;联合双端固定桥应力分布较为合理;联合单端固定桥出现明显的应力集中,应力分布不均。结论天然牙-种植体联合固定修复(TISBs)应尽量避免单端固定桥设计,而联合双端固定桥中种植体的应力分布较为均匀;增加天然牙数目能改善联合固定桥中种植体侧的应力分布,但必须要避免或最大限度地减小侧向力。Objective To examine the effect of the different planting sites and natural teeth on stress distribution in implant-natural tooth supported denture for the case of mandibular posterior free-end edentation. Methods A threedimensional finite element model simulated a left mandibular posterior tooth-implant supported rigid fixed bridge and cantilever bridge and corresponding periodontal substance with computed tomography(CT) scan images combined with Mimics software, UG software and Ansys software. The vertical and oblique loading were applied to each model,and the Von Mises stress distribution was investigated. Results Von Mises stress is unevenly distributed in joint cantilever fixed bridge,and the concentration of the stress is more significant than that in joint rigid fixed bridge. Conclusion The joint cantilever fixed bridge should be avoided,and the joint rigid fixed bridge should have a more balanced support.Adding a natural abutment can improve stress distribution on the implant side of the fixed bridge supported by tooth-implant,the oblique force on the bridge should be avoided or reduced as possible.
