摘  要:目的探讨连续顺序追踪超声法在早孕期胎儿肢体规范化筛查中的应用价值及声像图特征。方法选取来本院行11~13^(+6)周早孕期筛查的孕妇11 056例,根据患者产后及引产后尸解结果,统计超声筛查早孕期胎儿肢体畸形的检出率,并评价超声用于早孕期胎儿肢体畸形筛查的敏感性、特异性、阳性预测值和阴性预测值。结果 11 056例孕妇在胎儿分娩或引产后病理解剖证实合计出生缺陷125例,出生缺陷发生率1.13%。其中肢体畸形23例,总发生率0.28%,占出生缺陷发生率18.4%。23例肢体畸形患儿中,早孕期(11~13^(+6))孕周诊断18例,占78.26%,中孕期诊断4例,占17.39%。1例并指在产后检出。早孕期联合中孕期诊断检出率为95.65%(22/23)。诊断敏感性:78.26%、特异性:100%、阳性预测值100%、阴性预测值99.95%。结论孕早期胎儿四肢超声规范化结构筛查可及早发现胎儿肢体畸形,更早决定妊娠进程,对于优生优育具有重要意义,并且超声诊断胎儿肢体畸形安全、简单、方便、无创、重复性好,是一种值得推广和普及的诊断方法。Objective:To investigate the application value and sound image characteristics of sequential sequential tracking ultrasound in the standardized screening of fetal body during early pregnancy. Methods:A total of 11 056 pregnant women who were screened from 11 -13^+6 weeks of pregnancy were selected,according to the results of the patients with postpartum and autopsy after induced labor statistics ultrasound screening for early pregnancy rate of fetal limb deformities,and evaluate ultrasound used in early pregnancy fetal limb deformities screening of sensitivity,specificity,positive predictive value and negative predictive value. Results:A total of 125 cases of birth defects were confirmed by pathological dissection of 11 056 pregnant women after childbirth or induced labor,and the birth defect rate was 1.13%. Of the 23 cases,the total incidence rate was 0.28%,accounting for 18.4% of the birth defects. In the 23 cases of limb malformation,18 cases were diagnosed in the early pregnancy(11 ~13^+6),accounting for 78.26%,and 4 cases were diagnosed during pregnancy,accounting for 17.39%.1 case and point to postpartum detection. The detection rate of prenatal diagnosis was 95.65%(22/23). Diagnostic sensitivity:78.26%,specificity:100%,positive predictive value 100%,negative predictive value 99.95%. Conclusion:Pregnancy early fetal limb ultrasound screening the canonical structure can be detected early fetal limb deformities,early decision process of pregnancy,for it is of great significance of eugenic and superior nurture,and ultrasonic diagnosis of fetal limb deformities safe,simple and convenient,noninvasive,repeatability is good,is a worthy of promotion and popularization of diagnostic methods.
