摘 要:目的探讨钙视网膜蛋白(CR)在新生儿先天性巨结肠(HD)中的表达及其意义。方法选择10例新生儿因其他非肠神经节病变而行结肠手术的患儿结肠全层的病理标本作为对照组,50例新生儿HD狭窄段结肠肠壁作为实验组,分别行CR、神经细胞特异性烯醇化酶(NSE)、S-100蛋白(S-100)免疫组织化学染色,显微镜下观察染色表达情况。结果 CR在对照组肠壁神经节细胞及神经纤维内表达强阳性,即正常肠壁黏膜下层和肌间层神经丛神经节细胞内呈棕黄色,弥漫分布于细胞胞浆,结肠黏膜细胞及肠壁肌细胞不着色;而CR在新生儿HD患儿痉挛段表达缺失。实验组患儿CR的检出率为100%,明显高于NSE检出率的86%和S-100检出率的94%,差异有统计学意义(P<0.05)。结论通过CR免疫染色标记新生儿HD肠壁,对其诊断的敏感性大大提高,CR免疫染色可以作为新生儿HD诊断的一个辅助指标。Objective To explore the expression of Calretinin in neonatal Hirschsprung and analyzed the significance. Methods The pathological specimens of the colon of 10 newborns, who underwent colonic surgery due to non - intestinal ganglia lesions, were selected as the control group. The pathological specimens of the narrow - segment colon of 50 newborns with Hirschsprung' s disease were selected as the experimental group. The expression of Calretinin (CR), Neurone specific enolase(NSE) and s - 100 protein ( s - 100) was observed'under the microscope by using immunohistochemical staining technique. Results Calretinin immunohistochemical staining was strongly positive in gangliocyte and nerve fiber of colon wall. It means that positive shows claybank, which distributeddiffusely in cytoplasm of ganglionic cells both in the myenteric and submucosal plexuses. The colonic mucosa cells and muscle cells do not stain. The CR expression lost in the narrown segment of Hirschsprung. In experiment group,the relevance rate of CR for neonatal Hirschsprung' s disease was 100%. It' s apparently higher than NSE (86%) and S - 100 (94%). The difference was statistically significant (P 〈 0.05). Conclusions CR immunohistochemical staining improve significantly the sensibility of diagnosis of Hirschsprung. It can used to be a assistant? index for the diagnosis of neonatal Hirschsprung.
