Objective To explore strategies for revision of Asian blepharoplasty. Methods From 2012 to 2017, 103 patients dissatisfied with their double eyelid surgeries were admitted to our hospital for repair of flaws, including multiple creases, loss of double eyelid, and asymmetric double eyelid. The causes of patient dissatisfaction with double eyelid surgery were analyzed. Results Of the103 patients, 2 cases still had mild bilateral axillary asymmetry postoperatively. One of these cases obtained satisfactory results after frontalis suspension, and the other patient was still in follow-up. The remaining 101 patients all had satisfactory results after operation. Conclusion Repair of an unsatisfactory double eyelid is based on a better understanding of the mechanisms of surgery and the formation of the double eyelid. Creating a proper surgical strategy is also a very key point in tackling these situations. Surgical restoration includes release of the adhesion from the former operation, restoration of the normal anatomy of the eyelid, correction of the unsatisfactory double eyelid based on the specific reason as well as fixation of the incision line on the ideal position of the supratarsal area.
