摘　　要：目的介绍一种真皮瓣固定法矫治上睑皮肤松弛的术式。方法对诊断为上睑皮肤松弛或上睑臃肿的24例患者,设计眉下皮肤切除并形成真皮瓣,去除多余的眼轮匝肌或悬吊眶隔,缝合眼轮匝肌,将真皮瓣固定于眉上约3 mm深面的额肌上,缝合切口。结果术后随访3个月至2年,所有患者上睑皮肤松弛或上睑臃肿得到改善,切口隐蔽,瘢痕不明显,眉部表情自然。结论眉下切口真皮瓣内固定法可应用于矫治老年女性的上睑皮肤松弛、上睑臃肿,也适合年轻人上睑臃肿的改善治疗。此术式符合上睑正常解剖特点,术后效果确切,切口隐蔽,瘢痕不明显。Objective To introduce a method of correction of upper eyelid skin laxity with dermal flap fixation. Methods A total of 24 cases were diagnosed with relaxed or bloated upper eyelid skin. The designated skin below the eyebrow was excised and the dermal flap was developed. The excess orbicularis muscle or orbital septum was removed. Then the dermal flap was sutured to the surface of the frontal muscle, approximately 3 mm deep on the eyebrows. Results Patients were followed up for 3 months to 2 years. During that time, upper eyelid skin laxity or upper eyelid swelling in all patients was improved. The incision was concealed, the scar was not obvious,and the contour of the brow was natural. Conclusion Incision below the eyebrow and dermal flap fixation can be used to treat upper eyelid skin laxity and upper eyelid swelling in aging women. It is also suitable for the treatment of younger patients with bloated upper eyelids.This method results in a normal anatomy of the upper eyelid. The incision is concealed and the scar is not obvious.
