彩色多普勒超声检查在妇科肿瘤诊断中的临床价值

摘　　要：目的探讨彩色多普勒超声检查对妇科肿瘤诊断的效果。方法 80例妇科肿瘤患者入院后行彩色多普勒超声检查,间隔1 h后,进行CT检查,根据彩色多普勒超声及CT检查结果,计算联合检查方式的灵敏度、特异度及准确性,以此评价彩色多普勒超声在妇科肿瘤诊断中的价值。结果以病理切片诊断为金标准,CT诊断确诊43例为良性肿瘤,27例为恶性肿瘤,漏诊7例,误诊3例,CT诊断灵敏度86.0%,特异度90.0%,诊断符合率87.5%;经彩色多普勒超声诊断,确诊40例为良性肿瘤,25例为恶性肿瘤,漏诊10例,误诊5例,彩超诊断灵敏度80.0%,特异度83.3%,诊断符合率81.3%。彩超诊断灵敏度、特异度、诊断符合率与CT相比,无统计学差异(P>0.05)。彩色多普勒超声与病理诊断经一致性检验技术,K值为0.768;CT与病理诊断经一致性检查,K值为0.837,CT与病理诊断一致性较高。结论与CT检查相比,彩色多普勒超声对妇科良恶性肿瘤诊断准确性相对较低,但该检查方式无创、便捷、无射线,是临床诊断妇科肿瘤的理想检查方法。Objective To explore the clinical value of color Dopper ultrasound in the diagnosis of gynecological tumor.Methods 80 cases of gynecological cancer patients received color Doppler ultrasound examination,interval 1 h,and the CT examination. The sensitivity,specificity and accuracy of the combined examination were calculated according to the results of color Doppler ultrasonography and CT. The value of color Doppler ultrasound in the diagnosis of gynecological tumors was evaluated.Results Pathological diagnosis as a standard,CT diagnosis confirmed 43 cases of benign tumors,27 cases of malignant tumors,missed 7 cases,misdiagnosis in 2 cases. The sensitivity of CT diagnosis was 86. 0%,the specificity was 90. 0%,and the diagnostic coincidence rate was 87. 5%. Color Doppler ultrasound confirmed 40 cases of benign tumors,24 cases of malignant tumors,missed10 cases,misdiagnosis in 5 cases. The sensitivity of color Doppler ultrasound was 80. 0%,the specificity was 83. 3%,and the diagnostic coincidence rate was 81. 3%. There was no significant difference in diagnostic sensitivity,specificity and diagnostic accuracy between CT and color Doppler ultrasound（ P 〉 0. 05）. The color Doppler ultrasound and pathological diagnosis by the consistency check,K value was 0. 768. CT and pathological diagnosis by the consistency check,K value was 0. 837. CT and pathological diagnosis had high consistency. Conclusion Compared with CT examination,color Doppler ultrasound diagnosis of benign and malignant gynecological diagnosis is relatively low. But color Doppler ultrasound is noninvasive,convenient,no rays,it is the ideal clinical diagnosis method of gynecological tumors.
