射频消融、TACE及联合治疗结直肠癌肝转移的效果比较

摘　　要：目的比较肝动脉栓塞化疗(TACE)、射频消融(RFA)及联合治疗结直肠癌肝转移的效果。方法将120例结直肠癌肝转移患者为3组,每组40例。TACE组给予TACE,RFA组行RFA治疗,联合组在TACE4周后给予RFA治疗。比较3组疗效。随访3 a观察患者生存情况。结果 TACE组与RFA组的有效率差异无统计学意义(P>0.05)。联合组较其余2组有效率均明显提高,差异有统计学意义(P<0.05)。治疗1 a后,3组患者生存率差异无统计学意义(P>0.05)。治疗2、3 a后,联合组较其余2组生存率明显升高,差异有统计学意义(P<0.05)。结论 RFA联合TACE治疗结直肠癌肝转移,疗效优于单一TACE治疗或RFA治疗,并明显改善患者术后的生存质量。Objective Comparison of transcatheter arterial chemoembolization（ TACE）,radiofrequency ablation（ RFA） and combined treatment for liver metastases from colorectal cancer. Methods Totally 120 patients with liver metastases from colorectal cancer were divided into 3 groups,40 cases in each group. Group TACE was given TACE and RFA group for RFA treatment,and the combined group was given RFA treatment after TACE 4 weeks. The effects of the 3 groups were compared. The patients were followed up for 3 years to observe the survival of the patients. Results There was no significant difference in the effective rate between group TACE and group RFA（ P 〉 0. 05）. The effective rate of the combined group was significantly higher than that of the other 2 groups,with a statistically significant difference（ P 〈0. 05）. After 1 years treatment,there was no significant difference in the survival rate between the 3 groups（ P 〉 0. 05）. After 2 year and 3 year treatment,the survival rate of the combined group was significantly higher than that of the other 2 groups（ P 〈 0. 05）. Conclusion RFA combined with TACE in the treatment of liver metastases from colorectal cancer is better than single TACE or RFA treatment,and significantly improves the postoperative survival rate.
