中药熏蒸与冲击波联合治疗脑卒中后偏瘫肩痛疼痛改善及上肢功能康复疗效观察

摘　　要：目的：探讨中药熏蒸与冲击波联合治疗脑卒中后偏瘫肩痛的疼痛改善及对上肢功能康复疗效的影响。方法：选取2016年9月—2017年6月常州市德安医院康复中心收治的90例脑卒中后偏瘫肩痛患者,按数字表法随机分为中药熏蒸组、冲击波组以及中药熏蒸与冲击波联合治疗组各30例,3组在给予常规康复训练,另分别采取中药熏蒸治疗、冲击波治疗以及中药熏蒸与冲击波联合治疗,3组均治疗20 d为1个疗程,完成1个疗程后进行疼痛疗效评估,上肢功能康复（Fugl-Meyer）与生活质量（SF-36）评估。结果：在1个疗程结束时,3组患者VAS评分较治疗前均有明显降低（P〈0.05）,其中,联合治疗组VAS评分显著低于中药熏蒸组与冲击波组（P〈0.05）,冲击波组VAS评分明显低于中药熏蒸组（P〈0.05）;联合治疗组疼痛治疗有效率达到96.7%,明显高于中药熏蒸组的73.3%与冲击波组的86.7%（P〈0.05）;联合治疗组患者疗程结束后Fugl-Meyer上肢评分明显高于中药熏蒸组与冲击波组（P〈0.05）;联合治疗组患者在躯体功能、疼痛、精神状态方面改善明显优于其他两组（P〈0.05）。结论：中药熏蒸与冲击波联合方案治疗脑卒中后偏瘫肩痛,其疗效优于单一中药熏蒸治疗与单一冲击波治疗,并且可明显提高患者的上肢运动功能以及生活质量。Objective ：To explore the traditional Chinese medicine fumigation with shock wave therapy after stroke hemiplegic shoulder pain pain improved and for the effect of upper limb function rehabilitation. Methods ：A total of 90 cases from Sep. 2016 to June 2017,Changzhou De＇an hospital rehabilitation center after stroke patients with hemiplegic shoulder pain,according to the digital table method were randomly divided into Chinese medicine fumigation group,shock wave group and Chinese medicine fumigation with shock wave treatment group（30 cases each,three groups were given routine rehabilitation training,another respectively adoptted traditional Chinese medicine fumigation therapy,and combination treatment with shock wave therapy and Chinese medicine fumigation and shock wave,the three groups were treated 20 d for a period of treatment,pain assessment after completed a course of treatment,upper limb function rehabilitation（Fugl-Meyer）and evaluation of quality of life（SF-36）. Results ：At the end of a period of treatment,VAS score in three groups of patients were significantly lower than before treatment（P〈0.05）,among them,the combination group VAS score was significantly lower than that of traditional Chinese medicine fumigation and shock group（P〈0.05）,VAS score shock wave group was obviously lower than that of traditional Chinese medicine fumigation group（P〈0.05）;Pain treatment efficiency reached 96.7%,combined treatment group was obviously higher than that of traditional Chinese medicine fumigation with 73.3% of the shock wave group 86.7%（P〈0.05）;Combined treatment group after treatment in patients with Fugl-Meyer upper score significantly higher than that of traditional Chinese medicine fumigation and shock group（P〈0.05）;Combined treatment group patients in physical function,mental status of pain,improved obviously superior to other two groups（P〈0.05）.
