急性阑尾炎与正常阑尾的CT影像特点及其临床诊断分析

摘　　要：目的分析急性阑尾炎与正常阑尾的CT影像特点及其临床诊断价值。方法对2015年4月至2017年4月收治的40例急性阑尾炎患者的临床资料进行统计分析，将这些患者设为急性阑尾炎组，另选取同期接收的进行健康体检的40例正常阑尾人员设为正常阑尾组，对两组人员阑尾的显现情况及腔内物形态、阑尾及其周围的螺旋CT表现情况进行统计分析。结果急性阑尾炎组患者阑尾的显现率95．0％（38／40）显著高于正常阑尾组82．5％（33／40）（P〈0．05），腔内物形态固体比例20．0％（8／40）显著高于正常阑尾组100％（4／40）（P〈0．05），层数显著多于正常阑尾组（P〈0．05），直径、管壁厚度均显著长于正常阑尾组（P〈0．05），40例急性兰尾炎患者中，脂肪条带33例，局部积水8例，脓肿形成5例，分别占总数的82.5％、20．0％、12．5％，均显著高于正常阑尾组7．5％、25％、0（P〈0.05）。结论急性阑尾炎与正常阑尾的CT影像特点差异较大，具有较高的临床诊断价值。Objective To analyze the CT imaging features and clinical diagnostic value of acute appendicitis and normal appendix. Methods The clinical data of 40 cases of acute appendicitis in our hospital from April 2015 to April 2017 were analyzed. These patients was taken as acute appendicitis group. Another 40 cases of normal appendix of the same time were taken as the normal appendix group. Appearance of the appendix, cavity morphology, spiral CT and its surroundings appearances of the appendix of the two groups were analyzed. Results The acute appendicitis patients appendix appearance rate 95% （38/40） was significantly higher than that of normal appendix in group＇ s 82.5% （33/40） （P〈0.05） . 20% of the proportion of solid matter shape cavity （ 8/40 appendix 10% ） was significantly higher than that in normal group （ 4/40 ） （ P〈0.05 ） . The number was higher than that in the normal group （ P〈0.05 ） . Appendix diameter and wall thickness were significantly larger than those in the normal group （ Appendix P〈0.05 ） . Among 40 cases of with acute appendicitis, 33 cases were fat strip, 8 cases of local water, 5 cases of abscess, which accounted for 82.5%, 20%, 12.5%, significantly higher than those of normal appendix group＇ s 7.5%, 2.5%, 0 （P〈0.05） . Conclusion The CT imaging features differences between acute appendicitis and normal appendix are distinct, showing high clinical diagnosis value.
