血清PECAM-1在Ⅳ期胃癌中的表达及意义

摘　　要：[目的]研究血清PECAM-1表达与胃癌患者预后的相关性,了解其在胃癌发生发展过程中的作用。[方法]入组初诊的Ⅳ期胃癌患者56例,经组织病理确诊、符合本研究入组标准,于入院后未经治疗前采取外周血5ml,运用酶联免疫法（ELISA法）检测外周血中PECAM-1蛋白的表达,分析胃癌患者外周血中PECAM-1蛋白表达水平与预后及临床病理特征的关系。[结果]56例患者的中位总生存时间（m OS）为446d。胃癌患者血清PECAM-1中位值为79.5ng/ml,以该值作为分界值将患者分为高浓度组和低浓度组,PECAM-1高浓度组的m OS低于PECAM-1低浓度组（360d vs 558d,P=0.012）。PECAM-1高浓度组与低浓度组间患者性别、年龄（≥60岁与〈60岁）、一线化疗方案（XELOX与SOX）及周期数（≤4周期与〉4周期）、是否接受二线以上化疗差异均无统计学意义,但PECAM-1低浓度组一线化疗疗效优于高浓度组（P=0.043）。[结论]Ⅳ期胃癌患者血清PECAM-1水平与一线化疗疗效及预后生存可能相关,PECAM-1水平高者预后差,一线化疗疗效差。[Objective] To analyze the relationship between serum PECAM-1 levels and the prognosis in patients with stage Ⅳ gastric cancer. [Methods] Fifty-six patients with stage Ⅳ gastric cancer were enrolled. The serum levels of PECAM-1 was detected by enzyme-linked immunosorbent assay（ELISA）. And the relationship of serum PECAM-1 levels with clinical features and survival was analyzed. [Results]The median overall survival time（m OS） of patients was 446 d and the median serum PECAM-1 level was 79.5 ng/ml. The OS in patients with high PECAM-1 level（79.5 ng/ml） was lower than that in those with low PECAM-1（≤79.5 ng/ml）（360 d vs. 558 d,P=0.012）. There were no significant differences in gender,age（≥60 years or 60 years）,the first-line chemotherapy regimens（XELOX or SOX） and the number of cycles（≤4 cycles or 4 cycles）,and receiving further chemotherapy between patients with high and low PECAM-1 levels,but the efficacy of first-line chemotherapy in patients with low PECAM-1 was better than that in patients with high PECAM-1（P=0.043）. [Conclusion] The serum PECAM-1 level may be related to the efficacy of first-line chemotherapy and the prognosis in patients with stage Ⅳ gastric cancer.
