摘　　要：目的探究异基因造血干细胞移植后血清可溶性白介素-2受体(soluble interleukin-2receptor,sIL-2R)对急性移植物抗宿主病(acute graft-versus-host disease,aGVHD)的预测价值。方法回顾分析我院接受异基因造血干细胞移植患者12例,其中6例发生aGVHD,6例未发生aGVHD。移植前、移植后+14天、+28天、+42天、+60天、+90天通过ELISA试剂盒检测患者外周血sIL-2R水平,并分析其与aGVHD发生的相关性。结果本组患者血清sIL-2R水平为986-6957pg/ml。发生aGVHD与未发生aGVHD的患者移植前、移植后+14天及+90天血清sIL-2R有明显差别(均P<0.05)。移植后第+14天血清sIL-2R预测aGVHD的cutoff值为3658pg/ml,ROC曲线下面积为0.95,敏感性100%,特异性80%(P=0.027)。结论异基因造血干细胞移植后早期sIL-2R水平的升高与移植后aGVHD的发生相关,移植后+14天sIL-2R水平可能作为预测临床aGVHD发生的重要指标之一。Objective To study the predictive value of serum soluble interleukin-2 receptor for acute graft-versushost disease（aGVHD）after allogeneic hematopoietic stem cell transplantation（allo-HSCT）.Methods Twelve patients who underwent allo-HSCT were retrospectively analyzed in this study.Six patients suffered from aGVHD,and the remaining 6 patients had no signs of aGVHD.Serum sIL-2 Rlevels were monitoring at baseline and on ＋14 day,＋28 day,＋42 day,＋60 day,＋90 day after allo-HSCT.The association between sIL-2 Rand aGVHD was evaluated.Results The serum sIL-2 Rlevels were 986-6957 pg/ml in this study.sIL-2 Rlevels of patients with aGVHD were significantly higher than those without aGVHD at baseline and on＋14 day and＋90 day after allo-HSCT（all P〈0.05）.The cutoff value for prediction aGVHD on ＋14 day was 3658 pg/ml,with area under curve 0.95.The sensitivity was100%,and the specificity was 80%（P=0.027）.Conclusion Serum sIL-2 Rlevels early after allo-HSCT were associated with aGVHD,and sIL-2 Rlevels on ＋14 day after allo-HSCT may be an important index for prediction of clinical aGVHD.
